Tailbone pain (coccyx dard) is a sharp or deep ache right at the bottom of the spine that flares when you sit, and worst of all when you stand up from sitting. Most cases follow a fall, childbirth, or long hours on a hard seat or motorbike, and most settle over weeks to a few months once you take the pressure off with the right cushion and the right way to sit.

What is the coccyx, and why does it hurt so much when you sit?
The coccyx is a small tail of three to five tiny bones fused together at the very bottom of the spine. When you sit upright, your weight rests on your two sitting bones, but the moment you slump or lean back you roll onto the tailbone, and that small, poorly padded tip suddenly takes a load it was never shaped to carry. Hence the pain.
That is why the pain has such a clear pattern. A soft sofa that you sink into, a slouched posture on a charpai, or leaning back in an office chair all push the coccyx into the seat. Ouch. Standing up hurts because the muscles of the pelvic floor, which attach to the coccyx, pull on it as you rise.
Common causes of tailbone pain in women and men
- A fall onto the bottom. Slipping on a wet bathroom floor or stairs is the classic cause. The coccyx can be bruised, sprained, or less often fractured.
- Childbirth. The baby’s head presses the coccyx backwards during a vaginal delivery. Pain after birth is common and usually eases over weeks.
- Pregnancy. Hormones loosen the joints of the pelvis, and the growing bump changes posture. Tailbone pain during pregnancy is common and usually not a warning sign.
- Long sitting on hard seats. Office chairs, rickshaws, and especially motorbike seats on bumpy roads, where each jolt goes straight up the spine.
- Floor sitting. Sitting cross-legged or with legs out, rolled back onto the tailbone, for long meals, sewing or prayer gatherings.
- Weight changes. Rapid weight loss leaves less padding; extra weight tilts the pelvis back when sitting.
- No clear cause. In a good share of people nothing obvious started it.
Women get it more often than men, mainly because a wider pelvis leaves the coccyx more exposed when sitting and because childbirth pushes on it directly, which is also why so many mothers notice it for the first time in the weeks after a delivery.
How to relieve tailbone pain from sitting
Pressure is the enemy. Take weight off the coccyx while it settles. These are the steps that usually help most, in order.
- Get a cut-out cushion. A foam wedge with a U-shaped gap at the back leaves the tailbone hanging in air. Place it with the gap at the back. It beats a round donut ring for most people, because donuts can still press the tip.
- No cushion yet? Fold a thick towel into a strip and sit on two of them placed under each thigh, with a gap in the middle under the tailbone.
- Lean forward slightly. Sit on the sitting bones with a small forward tilt, feet flat. Avoid slumping back on sofas.
- Stand up the smart way. Scoot to the edge of the seat, lean forward, and push up with your legs and hands rather than rocking back.
- Break the sitting. Stand or walk for 2 to 3 minutes every 30 minutes. Set a phone timer if you work at a desk.
- Cold then warm. For the first 2 days after a fall, a cloth-wrapped ice pack for 15 minutes, a few times a day. After that, a warm water bottle for 15 to 20 minutes eases muscle spasm.
- Keep stools soft. Straining on the toilet pulls on the coccyx. Water, fibre and a footstool under your feet in the toilet help.
Floor sitting and motorbike tweaks
On the floor, sit on a folded cushion so your hips are higher than your knees and lean slightly forward rather than back, which matters most at long dawats, Quran khwani gatherings and sewing sessions where people sit for an hour or more without shifting. Shift often. Sitting to one side with legs folded, or on a low peerhi (stool), is often easier. For motorbikes, a gel or thick foam seat cover helps, and so does slowing down over speed breakers and standing slightly on the footrests over bad patches. A few weeks off the bike, if you can, speeds things up.
Gentle stretches for tailbone pain
Once the sharp stage has passed, these ease the tight muscles around the coccyx. Lying on your back, hug one knee to your chest and hold 20 to 30 seconds, 3 times each side. Then the child’s pose: kneel, sit back towards your heels and reach your arms forward, 3 holds of 20 seconds. Stop anything that sharpens the pain.
Tailbone pain after childbirth and in pregnancy
After a delivery, pain often shows up in the first days when sitting to feed. Use the cut-out cushion, feed lying on your side, and use the stand-up method above. Most improve over 6 to 12 weeks. In pregnancy, side-lying with a pillow between the knees, a wedge cushion, and avoiding long slouched sitting help. Pain relief? Ask your doctor or midwife first, gels included.
Many homes offer warm oil massage and a hot sek (warm compress) for back and tailbone aches, and after childbirth a warm, rested chilla. The warmth and rest are sensible. Deep pressure on a freshly injured tailbone is not; keep any massage gentle and on the muscles around it.
Myth: tailbone pain means the bone is broken and needs setting
Most tailbone pain is a bruise or strain, not a fracture. Even when a fracture shows up, the treatment is usually the same as above: cushion, time and pain relief. No cast. No sling. Manual “setting” by a pehlwan or bone-setter can hurt and is not needed.
Another common fear, from the searches people make, is cancer. Relax. Tailbone pain is almost always mechanical, a tumour here is rare, and when one does occur it usually brings other signs, listed below, rather than simple pain on sitting.
When is tailbone pain serious? Red flags
Go to emergency the same day if you have numbness around the bottom or inner thighs (saddle area), new trouble passing urine or stool, or loss of control of either, or new weakness in the legs. See a doctor soon if there is fever with pain, a red, swollen or leaking lump in the crease above the buttocks (possible pilonidal sinus), pain at night or at rest that doesn’t depend on sitting, unexplained weight loss, a history of cancer, blood in the stool, or pain that is not improving after 8 weeks.
