Anal Fissure vs Piles: Bawaseer Alamat and the Tearing Pain

A cutting, glass-like pain as the stool passes, then a burning ache that lasts an hour afterwards, with a streak of bright red on the tissue: most people call that bawaseer, but it is far more often an anal fissure, a small tear. Knowing the bawaseer alamat (piles signs) from fissure signs matters, because the two feel different, happen together after delivery, and are handled differently.

Bawaseer Alamat vs Fissure Signs

Piles (haemorrhoids) are swollen cushions of blood vessels in and around the anus. Everyone has these cushions; they become “piles” when they swell, bleed or slip out. A fissure is a split in the thin skin lining the anal canal, usually at the back, caused by a hard or very large stool stretching it.

The one exception to “piles don’t hurt much” is a thrombosed external pile. A clot forms in an outside pile, making a sudden, very tender, purple-blue lump. It hurts a lot. It needs a doctor within a day or two, since early on it can be released with a small procedure.

When bleeding needs more than a sitz bath

See a doctor soon for any rectal bleeding if you’re over 40, if the blood is dark or mixed through the stool, if there’s black tarry stool, if your bowel habit has changed for more than a few weeks, if you’ve lost weight without trying, feel tired and pale (possible anaemia), or have a family history of bowel cancer. Go to emergency for heavy bleeding, fainting or dizziness. Never apply caustic pastes, acids, “kala dhaga” chemical threads from a roadside seller, or burning powders to the anus; they cause deep burns, infection and scarring that narrows the opening. Pregnant or breastfeeding women should ask before using any medicated cream or laxative.

Why Fissures Keep Coming Back

Here’s the cycle that traps people. A hard stool tears the lining. The tear hurts, so the muscle ring around the anus (the internal sphincter) goes into spasm. Spasm reduces blood flow to the tear and keeps it from healing. Fear of the pain makes people hold stool in, which makes the next one harder and larger. It tears again.

Breaking that cycle is the whole job. Most fresh fissures (under about 6 weeks) settle with soft stools and warm baths. A fissure lasting longer than 6 to 8 weeks is called chronic, and that’s when a doctor adds prescription ointments that relax the muscle, or discusses a small procedure.

The Home Routine for Both

  1. Soften the stool. Aim for plenty of fibre: 2 to 3 servings of fruit (a guava, a pear, a handful of grapes) and 3 of vegetables a day, whole wheat atta or some chokar in the roti, dal and chana. If you use isabgol, take it as the pack directs with a full glass of water.
  2. Drink enough. Keep urine pale yellow. Fibre without water makes stools harder, not softer.
  3. Warm sitz bath. Sit in a clean tub or wide bowl of plain warm water (comfortably warm to the inner wrist, around 38 to 40 °C) for 10 to 15 minutes, two or three times a day and after each bowel movement. No salt, no Dettol needed.
  4. Don’t strain or sit long. Leave the phone outside. Five minutes, then get up and try later.
  5. Raise the feet. A small stool (patri or a stack of books) under the feet on a commode brings the knees up, which straightens the passage.
  6. Clean gently. Water from the lota, then pat dry. No rough tissue rubbing, no scented wipes.

A two-week “soft stool” test

For 14 days, keep a tiny chart: did you strain, was the stool soft, did it hurt? Add one fibre change at a time. If a fresh fissure isn’t clearly better after two weeks of truly soft stools, see a doctor rather than trying stronger totkay.

Bawaseer After Delivery

New mothers get both problems at once. Pregnancy hormones slow the gut, iron tablets harden the stool, the baby’s head presses on veins, and pushing in labour swells the cushions. Then come stitches and the fear of opening the bowels. That first bowel movement after delivery is often dreaded more than anything else in the first week.

What helps: drink with every feed, eat fibre from the first day home, sitz baths (which also soothe stitches), and press a clean folded pad gently against the stitches when passing stool. Ask your doctor or midwife for a stool softener that’s safe while breastfeeding; don’t take laxative teas or strong churans. How it was used

Unani and Ayurvedic practice has long treated bawaseer through diet first: more fibre, buttermilk, less hot spice, and warm water baths. Ayurveda also has ksharsutra, a medicated thread tied by trained surgeons in hospitals. That is a clinical procedure. A caustic paste or thread applied by an untrained seller at home is a different thing entirely and causes real harm.

What the Doctor Can Offer

For fissures that don’t settle: prescription ointments that relax the sphincter, sometimes a botox injection, and for stubborn cases a small operation (lateral internal sphincterotomy) with a high success rate in trained hands. For piles: banding, injection, or surgery for large ones. The doctor will usually examine gently and may suggest a colonoscopy if your age or symptoms call for it.
