Night Sweats (Raat Ko Paseena): Room, Sugar, Infection or TB?

Can night sweats be serious? Usually they’re the room, not the body: a hot night, a power cut, a heavy razai or a late spicy dinner. The sweats that matter soak the kameez and sheet in a cool room, night after night, and come with weight loss, fever, a cough or lumps. In Pakistan and India that combination means a TB test, which is free at government centres. Here’s how to tell the difference in one week, and what to do tonight.

Room or body? The honest first question

Sweating is how the body cools down, and on a humid 32 °C night in Karachi or Lucknow, with the fan off because the power went at midnight and the room holding the day’s heat, waking damp is normal for everyone in the house. That isn’t a symptom. It’s weather.

True night sweats are different. They soak through clothes and bedding so that you have to change, they happen when the room is comfortable, and they keep happening. The key question is simple: was anyone else in the room sweating?

How to investigate night sweats at home: the one-week diary

  1. Each morning, write down whether you sweated, and how much: damp, needed to change clothes, or had to change the sheet too.
  2. Note the room: fan or AC on, power cut, blanket or razai, window open.
  3. Note what you ate and drank after 8 p.m.: a heavy or spicy dinner, chai, coffee, alcohol.
  4. If you feel hot or shivery, check your temperature with a thermometer at night and first thing in the morning.
  5. Weigh yourself on the same scale, same time, at the start and end of the week, and note any cough, lumps in the neck, armpit or groin, or tiredness.
  6. If you take diabetes medicine that can cause low sugar, and the sweats wake you, check your sugar at around 3 a.m. on two nights.

After a week you’ll usually see a pattern. Sweats only on hot nights or after late meals point outwards. Sweats on cool nights with fever or weight loss point to the body, and to a doctor.

Night sweats causes, most common first

Heat, bedding and clothing

Polyester sheets, foam mattresses, a thick razai in a room that’s warmer than it used to be, and tight synthetic nightclothes all trap heat against the skin, so the body keeps sweating to get rid of it long after you’ve fallen asleep. Most common cause by far.

Late meals, spice, chai and alcohol

A heavy dinner close to bedtime raises body heat as it digests. Chilli, hot chai and caffeine late at night add to it. Alcohol widens blood vessels in the skin and often causes sweating a few hours into sleep.

Anxiety and nightmares

Stress hormones cause sweating. People often wake from a bad dream damp, with a racing heart and a dry mouth, and if this is the pattern the sweats tend to cluster in stressful weeks, around exams, money worries or a family illness, rather than arriving every single night.

Menopause and perimenopause

Hot flushes at night are very common in the years around the last period, often from the mid-40s. The home steps for this are in menopause skin, sleep and stress care, so they’re not repeated here.

Low sugar at night

People on insulin or sulfonylurea tablets for diabetes can have low sugar overnight, which causes sweating, nightmares, and a headache or confusion on waking. The 3 a.m. check shows it. Know the steps in sugar low symptoms and the 15-gram rule, and tell your doctor, who may adjust your plan. Don’t change your medicine yourself.

Medicines

Some antidepressants, some painkillers, hormone treatments and steroids can cause sweating. Ask your pharmacist which of your medicines can do this. Don’t stop any of them on your own.

Thyroid

An overactive thyroid causes sweating, weight loss despite a good appetite, a fast heartbeat, shaky hands and a feeling of being hot when everyone else is comfortable. One blood test finds it.

Infections, and why TB comes up

Many infections cause sweats while the fever breaks. The one that matters most in South Asia is tuberculosis (TB), which classically brings drenching night sweats, an evening fever, weight loss and a cough lasting more than three weeks. TB is common, it spreads to family members, and it’s fully treatable with a course of medicine from a TB programme.

When night sweats need a TB test

Get tested if night sweats come with any of these:

  • a cough for more than three weeks, especially with phlegm or blood
  • weight loss without trying
  • a fever that rises in the evening
  • swollen glands in the neck, armpit or groin
  • someone in the home has or recently had TB

Rarer causes, including some blood cancers such as lymphoma, also cause night sweats with weight loss and lumps. That’s a reason not to ignore these signs, not a reason to panic, because most people who get tested turn out to have something common and easily sorted, and finding out early is always the better position to be in. Test, don’t guess.

What to do tonight

A cooler night, step by step

Switch to a cotton sheet and loose cotton nightclothes. Keep the fan on or the window open, and swap the razai for a thin cotton blanket unless the room is cold. Finish dinner and chai at least two hours before bed. Keep a glass of water and a spare kurta by the bed so a sweat doesn’t mean a full wake-up.

  • Cut alcohol. Take the last chai or coffee in the afternoon.
  • If you’re on diabetes medicine that can cause lows, do the 3 a.m. sugar check and show the numbers to your doctor.
  • Keep the diary going for a second week if the first wasn’t clear.
When to see a doctor

Within a few days: drenching sweats more than twice a week for two weeks in a cool room, or night sweats with weight loss, fever, lumps, a cough over three weeks, or new bruising or bleeding. The same day: night sweats with a fever of 39 °C or higher, fever lasting more than 3 days, confusion, or low sugar that keeps happening. Skip herbal powders and supplements sold “for sweating”; they don’t address the cause and can delay a diagnosis. Chills without fever: the short answer

Shivering or chills without a temperature are usually the same outside causes: a cold room after sweating, a damp sheet, tiredness, hunger or anxiety. In people on diabetes medicine, chills with sweating can be low sugar. Occasionally chills are the first sign of an infection before the fever shows, so check your temperature again an hour later. Chills that come with a fever, or chills with sweats and weight loss, follow the same rules as above.

Myth: night sweats mean “garmi” in the body

Many homes blame body heat and reach for cooling drinks or a change of diet. Staying hydrated is fine, but labelling drenching sweats as “garmi” has kept many people with TB from being tested for months. If the sweats come with a cough, weight loss or fever, the test matters more than any drink.

Can night sweats be serious?

They can, when they soak your clothes in a cool room, happen night after night, or come with weight loss, fever, a long cough or lumps. Sweats only on hot nights or after late meals are almost always harmless.

First, check the room, bedding and late meals. If sweats continue every night in a cool room for two weeks, see a doctor for simple tests such as a blood count, thyroid, sugar and a chest X-ray or TB test.

How to stop night sweats naturally?

Cotton bedding and clothes, a fan, a lighter blanket, no heavy meals or chai in the two hours before bed, and less alcohol fix most room-related sweats. If they continue in a cool room, get tested.
