Knee Pain in Young Women: Floor Work, Squatting and Vitamin D
A 28-year-old who rolls roti on the floor, mops on her haunches, uses a squat toilet, stands three hours at the stove and hasn’t felt direct sun on her arms in a year is doing more to her knees than most 60-year-olds. Knee pain in young women (ghutnon ka dard) is rarely “arthritis at your age”. It’s load, position and weak muscles, often with low vitamin D underneath, and most of it can be changed at home with a stool, a pad, a pair of shoes and ten minutes of exercise.

Why does kitchen floor work hurt the knees so much?
Bend the knee fully and the kneecap is pressed hard into the groove at the end of the thigh bone. In a deep squat, the force across that joint is several times your body weight, and the menisci, the two crescent pads inside the knee, are pinched at their back edges. Hold that for twenty minutes cleaning daal or scrubbing a floor, twice a day, for years, and the cartilage behind the kneecap softens (chondromalacia) and the pads fray. The result: a dull ache around the kneecap, worse on stairs and after sitting, and a grinding feeling when you rise from the floor.
Kneeling is the other half. The bony point below the kneecap sits on a hard tile with your weight on it, the small fluid cushion in front of the kneecap gets squashed and inflamed, and the front of the knee swells into a soft, puffy bump. Cleaners’ and tilers’ knee, in a kitchen.
Squat toilets add a short, deep, daily dose of the same. One squat isn’t harmful; people who squat from childhood have strong, flexible knees. The problem is deep bending plus extra weight plus weak thighs plus an already sore kneecap, several times a day, with no recovery between.
Standing for hours: why the stove is as bad as the floor
Three hours at the stove looks like rest compared with squatting. It isn’t. Standing still on a hard floor loads the knee without the muscle pump that walking provides, so the joint fluid isn’t squeezed in and out, the calf doesn’t push blood back up, and the thigh muscles hold one position until they tire and hand the load to the ligaments. By evening the kneecap complains on the stairs. Bare feet or thin chappals on tiles pass every bit of that straight up the leg. Vitamin D: the part nobody in the house can see
Low vitamin D is close to universal among young women in Pakistan and northern India who spend the day indoors and go out covered. It matters for knees in two ways. Bone that isn’t getting enough vitamin D aches, deeply and vaguely, in the shins, thighs, hips and lower back, and that ache is often felt at the knee. And the thigh muscles that should protect the joint are weaker, so the kneecap tracks badly and the joint takes more of each step. Add the tiredness that comes with it and exercise feels impossible, which weakens the muscles further.
The tell-tale pattern is aching in both legs, worse at night and on rising, with tenderness when you press the shin bone, and trouble getting up from the floor without pushing on the thighs. Rising from a deep squat sends several times your body weight through the kneecap. Rising from a 30-centimetre stool sends a fraction of that. Same daal, different knee.
The swaps: same work, a fraction of the load
Nothing here needs a new kitchen. It needs a stool, a pad, and permission from the household to sit differently.
- Raise the floor work to a stool. A 30 to 40 centimetre wooden or plastic stool (peerhi or moora), knees bent no deeper than 90 degrees, feet flat. Roll, chop, clean daal and knead there. If the work must be on the ground, sit on a 10 centimetre cushion with one leg out straight and change legs every 10 minutes.
- Pad every kneel. A folded towel 5 centimetres thick, a kneeling pad, or a strip of old foam mattress under the knees for mopping and washing. Never bare knee on bare tile. Better still, a long-handled mop, standing.
- Break the squat. Squat toilet: hold a rail or the wall, go down over 3 seconds, and rise leading with the stronger leg. If the knees ache after, a commode chair that sits over the pan takes the squat away entirely.
- Shoes indoors. A cushioned closed shoe or a proper sandal with a firm heel cup, worn in the kitchen, not left at the door. Bare feet on tile is a load your knees pay for by evening.
- A mat at the stove. A 2-centimetre rubber or thick jute mat where you stand longest. Shift weight foot to foot, rise onto the toes 10 times every 20 minutes, and walk 2 minutes every half hour.
- Strengthen, alternate days, 10 minutes. Sit-to-stand from a firm chair, 8 repetitions, 2 sets. Straight-leg raise lying down, 10 each leg, 5-second hold, 2 sets. Side-lying leg lift, 12 each side. Wall sit, knees bent a quarter of the way, 15 seconds building to 45. Thighs working is right; sharp pain in the joint is stop. Judge at 6 weeks; most people feel the stairs change by then, and the full result takes about 12.
- Warm the evening knee, if you like. 2 teaspoons of mustard or sesame oil warmed in a katori over hot water, rubbed round the kneecap for 5 minutes at night, or a wrapped hot water bottle for 15 minutes. Patch test first, never on broken skin, never on a knee that’s hot and swollen. It relaxes the muscles and helps you sleep; it doesn’t do the work of the exercises.
Weight, periods and the knees
Young women gain weight after marriage and children for reasons that aren’t laziness: broken sleep, a diet built around roti and oil, no time and no permission to walk. The knee doesn’t care about the reasons. Every kilo is about 4 kilos through the joint per step, and 5 kilos gained is 20 extra kilos on each rise from the floor. Many women also notice the knees ache more in the days before a period. Ligaments loosen slightly under hormonal changes and fluid retention adds a little load, so the kneecap tracks worse for a few days. It’s real, it passes, and it’s one more reason the muscles need to be strong enough to hold the joint steady when the ligaments aren’t.
Myth: knee pain at 25 means early arthritis
Osteoarthritis is uncommon before 40 without an old injury. Kneecap pain in a young woman is almost always a tracking and load problem, and it’s one of the most fixable knee complaints there is. What isn’t a myth is that years of unchanged floor work, weight and weak thighs bring true wear forward by a decade or more, which is why the swaps matter now, not at 50.
Grandmothers answered aching young knees with a warm mustard oil malish at bedtime, a turmeric and salt paste tied on with cloth, and the order to sit with the legs stretched out (tangain seedhi karo). The malish and the stretched legs were sound; the paste is harmless warmth on unbroken skin. None of it addressed the stool, the shoes or the sun.
Try this for one week
Move only the roti-making to a stool and wear shoes in the kitchen, changing nothing else. Rate the evening knee ache out of 10 each night. Most women see the number drop within the week, which is the argument you’ll need when someone in the house says the stool looks lazy.
A hot, red, swollen knee with fever is a possible joint infection and needs a hospital that day. A knee that locks, gives way, or won’t take weight after a fall, calf pain with swelling (especially in pregnancy or after childbirth), or pain with numbness needs a doctor promptly. In pregnancy the swaps above are safe, but skip oils with camphor or strong essential oils and mention any swelling to your midwife. A limp or a swollen knee in a girl under 16 needs a doctor the same day. If you have diabetes, keep heat off any numb skin, and if you have kidney disease, high blood pressure or a history of stomach trouble, ask a pharmacist before using any painkiller gel or tablet. Why do my knees hurt at 25?
Almost always the kneecap tracking badly under load: deep squatting, kneeling on tiles, long standing, extra weight and weak thigh and hip muscles, often with low vitamin D underneath. It isn’t arthritis at that age without an old injury, and it responds to swaps and strengthening within 6 to 12 weeks.

Is squatting bad for the knees?
Not by itself. It’s harmful when it’s deep, prolonged, repeated many times a day, on a knee that’s already sore, in someone carrying extra weight with weak thighs. Shorter, supported squats with strong muscles are fine.
Can vitamin D deficiency cause knee pain?
Yes. Low vitamin D makes bones ache and weakens the thigh muscles that protect the knee, so pain around the joint is common, usually in both legs with night aching and tender shins. A blood test confirms it, and a doctor sets the correction from your level.
Should I stop using a squat toilet?
Not necessarily. Hold a rail, go down slowly and rise leading with the stronger leg. If the knees ache afterwards or you have a flare, a commode chair over the pan removes the deep squat without any building work.
Which exercise is best for knee pain in women?
Sit-to-stand from a chair and straight-leg raises build the front thigh, and side-lying leg lifts train the hip muscles that keep the kneecap straight. Alternate days, 2 sets each, judged at 6 weeks. Hip work matters more in women because the wider pelvis angles the thigh bone inward at the knee.
Does knee pain get worse before periods?
For many women, yes. Hormonal changes loosen ligaments slightly and fluid retention adds load, so the kneecap tracks worse for a few days. It passes with the period, and strong thigh and hip muscles make the difference smaller.
Is knee pain in pregnancy normal?
Common, because joints loosen and weight shifts forward. The stool, shoes, mat and gentle strengthening are all safe. Calf pain with swelling, a hot knee, or sudden severe swelling in pregnancy needs a doctor the same day.
