Wrist Pain From Phones and Typing: Carpal Tunnel or Strain?

Wrist pain from phones and typing is usually one of two things, and they need different fixes. Carpal tunnel is a squeezed nerve: numb, tingling thumb, index and middle fingers, worst at night, shaken out on waking. Tendon strain is an inflamed cord: pain at one spot on the wrist, worse with the movement itself, no numbness. Run the three self-checks, follow the branch that fits, and learn the signs that mean nerve tests, not another week of stretching.

Three self-checks that point one way or the other

None of these is a diagnosis. A doctor makes that. Together, they point you to the right branch.

  1. The dropped-wrist check (Phalen-type). Press the backs of both hands together in front of the chest, fingers pointing down, wrists fully bent, elbows level. Hold 60 seconds. Tingling or numbness creeping into the thumb, index and middle fingers within the minute points to carpal tunnel. Pain at the wrist with no tingling points away from it.
  2. The thumb-in-fist check. Tuck the thumb into the palm, close the fingers over it, and bend the wrist gently toward the little finger. A sharp pain on the thumb side of the wrist, right where a watch strap sits, points to the thumb tendons (de Quervain’s), the classic strain of new mothers and heavy scrollers. Once only. It’s a check, not an exercise.
  3. The map check. Trace the trouble with the other index finger. Numbness that includes the little finger is not carpal tunnel; it usually means the elbow nerve. One tender spot on the back of the wrist that worsens when you lift the fingers is the typing tendons. Pain that changes with neck position is the neck.

If it’s carpal tunnel: position, the night splint, nerve glides

The median nerve runs through a tight tunnel at the wrist with nine tendons for company. Anything that swells the contents squeezes it: bent wrists held for hours, repetitive gripping, pregnancy fluid, an underactive thyroid, diabetes. Three parts to the fix, in this order.

Stop bending the wrist. A big phone held in one hand, wrist cocked back, thumb doing all the work, is the modern cause. Hold it in one palm and tap with the other index finger. At a keyboard the wrists float level with the forearms, never resting on the desk edge. A rolled towel under the forearms fixes half of it.

The night splint, honestly. Most people curl the wrist under the pillow in sleep, which is why carpal tunnel wakes you at 3 am. A rigid splint (pharmacy or surgical shop, the kind with a metal bar holding the wrist straight) worn at night for 4 to 6 weeks helps a lot of people, especially early on and in pregnancy. What it won’t do: fix daytime symptoms unless you fix the daytime cause, or reverse wasting that has already begun. Night only. Straight, not tight. Six weeks of nights with no change means a doctor.

Nerve glides. Arm out to the side, palm forward, fingers up as if stopping traffic. Tilt the head slowly away from that arm until a gentle pull runs along the inner arm, then back. 10 slow repetitions, 3 times a day. A pull is right. Tingling? Smaller.

If it’s tendon strain: rest the movement, not the hand

Tendons are the cords that pull the fingers and thumb, running through slippery tunnels of their own. Repeat one movement a few thousand times, thumb scrolling or typing with the wrist cocked up, and the lining swells. The pain sits at the spot, with the movement, sometimes with a creak under a fingertip.

  1. Rest the one movement for 2 weeks. Thumb-side pain: no thumb scrolling, switch to the other index finger or voice notes. Back-of-wrist pain: no typing with the wrists cocked up. Everything else the hand does, keep doing.
  2. Ice, early. A wrapped ice pack on the spot, 10 minutes, 2 or 3 times a day for the first week. After that, warmth before stretching earns more than ice.
  3. Thumb stretch (de Quervain’s), from week 2. Thumb in the fist, wrist tipped gently toward the little finger until a mild pull, never a sharp pain. Hold 15 seconds, 5 repetitions, 3 times a day.
  4. Forearm stretches. Arm straight, palm down, press the fingers gently toward the floor with the other hand, 20 seconds. Palm up, draw the fingers toward the floor, 20 seconds. 3 rounds each, 3 times a day. A stretch along the forearm is right.
  5. Grip work, from week 3. Squeeze a rolled sock, hold 5 seconds, 10 repetitions, twice a day, then 10 rubber-band finger spreads. Tendons recover stronger when loaded gently, not when rested to death.

A fabric thumb spica (a splint holding the thumb still) helps de Quervain’s by day for the first 2 weeks if you must keep working. Longer than that makes it weaker.

Phone grip changes that fix both

Use two hands. That’s most of it. Rest the phone on the fingers of both, thumbs light, put a pop-out grip or a ring on the back so the fingers aren’t clamping to stop it slipping, scroll long reads with the other index finger and swap hands every 10 minutes, switch to voice typing for anything longer than a sentence, and park the elbow on a table or cushion so the wrist can stay straight instead of hovering. Every 20 minutes, open the hand wide, spread the fingers, and circle the wrists 5 times each way. Carpal tunnel in pregnancy

Fluid retention in the last three months swells the tunnel, so numb, tingling hands at night are common, often both hands, often with swollen ankles for company, and they usually settle within weeks of delivery once the fluid goes. Meanwhile the night splint is the first and safest step, hands raised on a pillow help, the nerve glides are fine, and hands tucked under the pillow are out. Say so at the antenatal visit. Swelling with headache, vision changes or a blood pressure of 140/90 or above is pre-eclampsia territory, urgent, and nothing to do with the wrist. New mothers who then get thumb-side pain from lifting the baby have crossed to the tendon branch: forearms under the baby, not hands hooked under the armpits.

When nerve tests are needed, and what they are

A nerve conduction study measures how fast a signal travels through the nerve at the wrist, and an EMG checks the muscles it feeds; neither hurts much, and neither is needed for mild, recent symptoms that answer to a splint. A doctor orders them when the picture is unclear, when the elbow or neck might be the real site, when 6 to 8 weeks of proper home care has changed nothing, or before deciding on a steroid injection or the small release operation. Ask for a review if the numbness is now constant rather than coming and going, if the pinch has gone weak (dropping keys, a coin you can’t pick up) or the fleshy pad at the base of the thumb looks flatter than the other side, because wasting means the nerve has been squeezed too long, if both hands are affected or the little finger is involved, or if 6 weeks of nightly splinting has changed nothing. A fall onto the outstretched hand is different again. That’s an X-ray, the same day, because a small wrist bone (the scaphoid) breaks quietly and mends badly when missed. Take care if

You have diabetes, an underactive thyroid, rheumatoid arthritis or kidney disease: carpal tunnel is commoner and slower to settle, so see a doctor earlier and never rely on a splint alone. A hot, red, swollen wrist with fever, or after a cut or thorn, may be infected and needs the same day. Painkiller gels and tablets carry their own rules in pregnancy, with ulcers, kidney disease and blood thinners, so the pharmacist chooses. Keep every binding loose enough to slip a finger under. How do I know if I have carpal tunnel or just wrist strain?

Can using a phone too much cause carpal tunnel?
