Prostate Badhna: Night Urination, PSA and Red Flags After 50

Getting up three times a night to pee, standing at the toilet waiting for a weak stream to start, then dribbling after: for men over 50, this is usually the prostate enlarging, and “prostate badhana kya hota hai” is one of the commonest questions they’re too shy to ask. One situation can’t wait for an appointment: if you suddenly cannot pass urine at all and the lower belly is swollen and painful, go to an emergency department now.

See a doctor urgently if

You can’t pass urine despite a full, painful bladder (acute retention): that’s an emergency, and a catheter brings immediate relief. Go the same day for urinary symptoms with fever, chills or burning (possible prostate or kidney infection), for visible blood in the urine, or if you’re passing much less urine than usual. Book soon for new bone pain in the back or hips, unexplained weight loss, or a strong family history of prostate cancer. Cold and flu medicines containing decongestants or sedating antihistamines can tip an enlarged prostate into retention; ask the pharmacist before buying them. Don’t start herbal “prostate” capsules alongside prescribed medicines without telling your doctor.

Prostate Badhana Kya Hota Hai?

The prostate is a walnut-sized gland below the bladder, wrapped around the tube (urethra) that carries urine out. In most men it slowly grows from middle age onwards. Quietly. Because it surrounds the tube, even a modest increase can squeeze the flow, much like a hand gently closing around a garden pipe. Size on the ultrasound and symptoms don’t always match, so a report that says “prostate 45 cc, enlarged” tells you far less than how many times you got up last night, how hard you strain and whether the stream stops and starts, and some men with large glands have few problems while others with smaller ones struggle every day.

BPH is not cancer. It doesn’t become cancer. But the two can exist together, and early prostate cancer usually causes no urinary symptoms at all, which is why the checks below matter.

The Symptoms, in Two Groups

Doctors split them into two groups, and it’s worth knowing which group bothers you most, because the questions, the tests and even the choice of medicine differ depending on whether the trouble is getting urine out or holding it in.

  • Emptying problems: slow start (hesitancy), weak stream, stop-start flow, straining, dribbling at the end, feeling the bladder isn’t empty.
  • Storage problems: needing to go often, urgency (can’t hold it), getting up at night (nocturia), occasional leaks.

Night urination has other causes too. Many. Drinking a lot of water or tea late, diabetes, heart failure, sleep apnoea and water tablets taken in the evening can all get a man out of bed, and none of them has anything to do with his prostate. A three-day bladder diary

For three days, write the time of each pee, a rough amount (use a measuring jug: small under 150 ml, medium, large over 300 ml), what you drank and when, and nights woken. It takes minutes a day and usually tells the doctor more than an hour of questions, because it separates a man who makes too much urine at night from a man whose bladder simply can’t empty properly, and those two need very different help. Plenty of men find their night trips start with the 10 pm chai.

PSA: What the Blood Test Can and Can’t Tell You

PSA (prostate-specific antigen) is a protein made by the prostate. Its level rises with prostate cancer, but also with BPH, infection, a recent ride on a bicycle or motorbike, recent ejaculation and a recent catheter or examination. So a raised PSA means “look further”, not “you have cancer”. And a normal one isn’t a guarantee.

Labs often flag above about 4 ng/mL, though doctors adjust for age and prostate size. Whether to test at all, especially without symptoms, is a decision to make with the doctor, weighing age, family history and what you’d do with the result.

  1. Avoid ejaculation for 48 hours before the test.
  2. Avoid long cycling or motorbike rides for 48 hours before.
  3. Don’t test during a urine infection; wait several weeks after it clears.
  4. Get repeat tests from the same lab when possible, so the numbers compare.

A raised PSA usually leads to a repeat test. Then maybe an MRI. Then a specialist.

Prostate Ayurvedic Upchar and Unani Remedies: Graded

Pharmacies, hakeem dawakhanas, Instagram sellers and the man with a loudspeaker at the Sunday bazaar all offer “prostate care” products, usually with a photo of a smiling grey-haired man and a promise of sleeping through the night within a week. Here’s the honest picture.

  • Saw palmetto: the best-known herbal. It isn’t a reliable help for urinary symptoms. Mostly harmless. Not a substitute.
  • Pumpkin seeds (kaddu ke beej): a good food. A handful as a snack is fine; there’s no reliable sign they shrink the prostate.
  • Gokshura, varun, punarnava: traditional Ayurvedic urinary herbs, mostly used as mild diuretics. More urine can mean more trips for a man whose outflow is already narrowed. Not helpful.
  • Multi-herb “prostate” capsules: unknown mixtures; some imported products have contained undeclared drugs. Tell your doctor about anything you take.

If a hakeem or vaid prescribes something, the amount is theirs to set, and the doctor should still know. Never delay a check because a herbal mix “seems to help”. Ever.

In the old books

Unani texts describe “usr-ul-baul” (difficult urination) in older men and advised warm sitz baths, avoiding cold, and not holding urine. Warmth and not holding it in are still sensible comfort measures, and they sit comfortably alongside modern checks, not in place of them.

What Helps Day to Day, and What the Doctor Offers

  1. Cut drinks, especially tea, coffee and cola, for 2 hours before bed, but drink enough earlier in the day.
  2. Try double voiding: pee, wait 30 seconds, then try again.
  3. Don’t hold urine for hours; go when you first feel the need.
  4. After peeing, press gently behind the scrotum and “milk” the tube forward to stop after-dribble.
  5. Keep stools soft; a full bowel presses on the bladder.

Doctors can prescribe medicines that relax the muscle in the prostate within days or slowly shrink the gland over months, which is a genuine option many men never hear about because they spent years on bazaar capsules first, and for larger or stubborn cases there are procedures such as TURP.
