Normal Weight but Big Belly: Why Your Waist Beats BMI for Men

Yes, you can be a normal weight but have a big belly, and for South Asian men a waist above about 90 cm (35.5 inches) is the line worth taking seriously, whatever the scale says. The scale counts every kilo the same. Your waist tells you where the kilos sit.

The short version

  • The number that matters most here: waist, measured at the belly, not the belt hole you have worn since your wedding.
  • Common guidance for South Asian men: waist about 90 cm or more means higher risk; a waist more than half your height says the same thing in a different way.
  • Why BMI misses it: BMI cannot tell fat from muscle, or belly fat from fat elsewhere.
  • What to check next: blood pressure, fasting sugar or HbA1c, a lipid profile and, if your doctor thinks it useful, liver tests.
  • Rule of thumb: a slim face and a tight belt is not a slim man.

How can my BMI be normal when my belly is big?

BMI is weight divided by height squared. That is all. It has no idea whether your 65 kg is mostly shoulders and legs or mostly sitting in front of your spine.

Take a worked example. A man who is 165 cm tall and weighs 65 kg has a BMI of roughly 23.9. On the standard international chart that reads as “normal”, and plenty of men stop reading right there, pleased. Now put a tape round his middle and it says 102 cm, the familiar 40 inches. That is about 12 cm over the 90 cm line that is commonly used for South Asian men. Same man. Two very different stories.

There is a second wrinkle. A WHO expert consultation found that many Asian populations carry a high risk of type 2 diabetes and heart disease at BMIs below the usual overweight cut-off of 25, and it proposed extra action points, including 23 and 27.5, for countries to use. So a BMI of 23.9 is not quite the all-clear for a Pakistani man that it might be for someone else. For a fuller look at those cut-offs, see our guide to BMI and waist size for South Asians.

What is visceral fat, in plain words?

There are two broad kinds of belly fat. One sits under the skin; you can pinch it. The other packs in deeper, around the liver, the gut and the other organs, and you cannot pinch it at all. That deeper layer is visceral fat.

A firm, round belly that pushes the shirt buttons forward, with fairly thin arms and legs, is the classic shape. Many desi men in their forties know it well: the “pot” that arrived somewhere between the third child and the promotion. Visceral fat is tied closely to insulin resistance, raised blood pressure, high triglycerides and fatty liver. It is the reason doctors care about the tape more than the scale.

The scale tells you how much. The tape tells you where. Where is the part that matters.

Why do South Asian men get diabetes and heart disease at lower weights?

Nobody has one tidy answer. What is well established is that, at the same BMI, South Asian adults tend to carry more body fat and more of it centrally than people of European descent. A large UK study following middle-aged adults found people of South Asian ancestry had roughly double the rate of heart attacks, strokes and related events over about 11 years, and the biggest contributors were blood pressure, diabetes and central fat. The same study found the usual risk calculators did not pick up that extra risk well.

That is why the NHS tells people from South Asian backgrounds that they face health problems at a slightly lower BMI, and why your waist is a better early warning than your weight.

How do I measure my waist properly?

  1. Stand up straight, shirt off or lifted, feet together.
  2. Find the bottom of your lowest rib and the top of your hip bone. The tape goes halfway between them, which for most men is around the belly button.
  3. Breathe out normally. Don’t suck in, and don’t puff out.
  4. Keep the tape level all the way round and snug, not digging in.
  5. Read it twice. Write it down with the date.

Trouser size is not a waist measurement. Most men wear their trousers below the belly, which is precisely why the trousers still fit. Our guide to measuring waist, weight and height at home has more on getting a reading you can trust.

My waist is over the line. What should I check next?

A big waist is a reason to look, not a diagnosis. Ask your doctor about these, and keep the results in one folder:

CheckWhat it showsLine to know
Blood pressureStrain on heart and arteries140/90 or more in clinic, about 135/85 at home
Fasting sugar or HbA1cDiabetes or prediabetesFasting 126 mg/dL or HbA1c 6.5% diagnoses diabetes
Lipid profileLDL, HDL, triglyceridesYour doctor reads it against your overall risk
Liver tests or scanFatty liverOnly if your doctor thinks it useful

If any of these come back raised, the plan changes from “would be nice” to “start this month”. Our lipid profile explainer walks through each number.

What brings the waist down fastest?

No exercise melts fat off one spot. Crunches build the muscle under the fat; they don’t remove the fat itself. The waist shrinks when total body fat comes down, and visceral fat often responds well once that happens. What actually moves it:

  • Less sugar in drinks. Chai with two spoons, five times a day, adds up quickly. Cutting it is often the single easiest win.
  • Smaller dinner, earlier. The big 10 pm meal followed by sleep is a habit many men can trim without missing much.
  • Walking most days, plus two short strength sessions a week (squats, push-ups against a wall, carrying shopping). Guidelines suggest at least 150 minutes of moderate activity a week.
  • Sleep. Short nights make the next day hungrier.
  • Not smoking, and that includes naswar.

The belt-hole test, upgraded

Measure your waist on the first Friday of every month, after Fajr, before breakfast. Same tape, same spot. Ignore the daily scale. If the tape number has not moved in two months, something in the plan needs adjusting, usually dinner size or chai sugar.

When to see a doctor first

Book a check before changing your diet if you take diabetes medicine (especially insulin or sulfonylureas, because eating less can push sugar too low), blood pressure tablets, or have kidney, thyroid or heart problems. Don’t start, stop or change any prescribed medicine on your own. Weight or waist falling without trying is a reason to see a doctor soon. Chest pain, pressure that spreads to the arm or jaw, or sudden breathlessness: call 1122 at once.

Normal weight but big belly: questions men ask

Is a big belly dangerous if my BMI is normal?

It can be. Fat stored deep in the belly is linked with diabetes, high blood pressure and heart disease even when BMI looks fine, which is why South Asian men are advised to watch the waist as well as weight.

What waist size is too big for a Pakistani man?

About 90 cm (35.5 inches) is the cut-off commonly used for South Asian men. A waist more than half your height is another warning sign. These are guidance lines, not a diagnosis.

Can I be skinny fat?

Yes. “Skinny fat” is the everyday name for normal weight with low muscle and a high share of body fat, often around the middle. Strength work plus less sugar is usually the fix.

Will sit-ups flatten my stomach?

No, not on their own. They strengthen the muscle underneath, but the fat on top only goes when total body fat comes down through food, walking and sleep.

Why is my belly big but arms and legs thin?

That is the typical central fat pattern. Many South Asian men store extra fat around the organs first, so the belly grows while the limbs stay slim.

Which blood tests should I get for belly fat?

Ask about fasting sugar or HbA1c, a lipid profile and blood pressure. Your doctor may add liver tests. There is no single “belly fat test”.

Does beer belly only come from beer?

No. Any regular surplus, whether sugary chai, sweets, fried snacks or large dinners, can build the same belly. The name is older than the science.

How quickly can visceral fat go down?

It often responds early once you lose weight steadily, but the pace varies. Aim for about 0.5 to 1 kg a week and judge the tape monthly rather than daily.

Should I worry if I have diabetes in the family?

Yes, take it as a nudge. A parent or sibling with type 2 diabetes plus a big waist is a good reason to get your sugar checked this year.
