Lactose Intolerance Weight Gain: Eating More Without Milk

If milk leaves you gassy, crampy or running to the toilet, you can still gain weight without it. Nuts, eggs, chicken, daal, oils and bananas carry the calories, and fermented dahi, lassi and ghee are often easier to tolerate than a full glass of milk.

In brief

  • Lactose intolerance: trouble digesting milk sugar, with bloating, gas and loose motions.
  • Milk allergy: an immune reaction, and a reason to see a doctor.
  • Often easier: dahi, lassi, ghee, hard cheese, lactose-free milk.
  • Do not: drop food groups without a reason or a test.

If you are losing weight without trying, or cannot gain despite eating, a doctor should look at that first. Poor absorption from coeliac disease can look like milk trouble, and so can irritable bowel; see coeliac disease and low weight.

Is it lactose intolerance or a milk allergy?

They are different problems. Lactose intolerance is a digestion issue, and the NHS describes symptoms such as tummy discomfort, wind, bloating and loose stools, which can start minutes or hours after eating dairy. A food allergy is an immune reaction and can be serious. Swelling of lips or face, hives, wheeze or breathing trouble after dairy means emergency care, not home experiments.

This article is not a diagnosis. A GP can arrange tests such as an elimination diet, a blood test or a breath test, and the NHS advises against cutting out foods without guidance from a GP or dietitian.

Which dairy foods are easier to handle?

Fermented and aged dairy usually have less lactose than milk, and many people tolerate them in small servings. Try one at a time.

FoodUsually
DahiOften fine in small servings
LassiOften fine, start with half a glass
GheeVery little lactose
Hard cheeseUsually low lactose
Lactose-free milkDesigned for this
Fresh paneer, milkMore likely to cause trouble

Start small with dahi

Fermented dahi and ghee carry far less lactose than milk. Try 2 to 3 spoons of dahi with lunch for three days, then a half katori. If symptoms stay away, build up slowly. For homemade dahi, see the dahi and lassi guide.

What replaces the calories from milk?

Milk usually adds around 120 to 150 kcal a glass, depending on fat. You can cover the gap with other foods that are easy to find.

  • A handful of mixed nuts and a spoon of peanut butter (add to roti or bananas).
  • Two eggs, boiled or as an omelette cooked in a spoon of oil.
  • Chicken, fish or qeema, with daal on other days.
  • Oil or ghee in measured spoons on roti, rice and daal.
  • Bananas, dates, potatoes and shakarkandi for extra energy.

Ideas for a bedtime snack that replaces the doodh habit are in bedtime snacks: doodh, kela and mewa. A longer list is in the desi high-calorie food list.

What about calcium if milk is out?

Dairy is a major calcium source for many families. If you cut it down a lot, ask your doctor or dietitian about other calcium sources and how much you need; that depends on age, sex and health, so it is theirs to set.

How do you know which food is the trigger?

Milk is the usual suspect, but bloating can also come from large portions, very fast eating, fried food, beans, or a sudden jump in fibre. Writing down what you ate and when symptoms began for a week is more useful than guessing. Take that note to a GP or dietitian, who can decide whether a test is needed. If symptoms appear with many different foods, that points away from lactose alone and is a good reason to be seen.

Do not swap milk for a pile of packaged “free from” products. Many are low in energy and protein and cost more. Plain food, such as eggs, daal, nuts and a measured spoon of ghee, usually does the job better and cheaper.

Eat slowly, keep portions modest and spread them across the day. Many people find that a small serving with a meal is easier on the gut than a large one on an empty stomach, so test dairy with food, not alone.

A simple plan for the first week

  1. Note what you ate and what happened in a short list on your phone.
  2. Replace milk with water-based drinks or lactose-free milk for three days.
  3. Reintroduce dahi and ghee first, a spoon or two at a time.
  4. Keep adding eggs, nuts and oils so calories do not drop.
  5. Book a GP visit if symptoms persist, weight keeps falling, or you see blood in stool.

If gas and acidity are the main issue, rather than only milk, see gentle foods for acidity and IBS.

See a doctor if

Symptoms follow dairy with swelling, hives or breathing difficulty, there is blood in the stool, you lose weight steadily, or a child is involved; children need a paediatrician, not a home plan. People with diabetes or kidney disease should ask a doctor or dietitian before changing protein and sweet foods. Avoid unlabelled weight gainer powders and herbal syrups, as some have been found to contain hidden steroids.
