Kamar Dard Ka Gharelu Ilaj: Lower Back Pain, the First 48 Hours
You bent to lift the atta sack, or stood up from the floor after a long dawat, and your lower back locked. Kamar dard ka gharelu ilaj (home care for lower back pain) is mostly about what you do in the next two days, and the biggest mistake is the one that feels most sensible: lying down and waiting.

What is usually going on
Most lower back pain is mechanical: a strained muscle, an irritated joint between two vertebrae, a disc that has bulged a little and pressed on something. It’s painful. It’s rarely dangerous. Scans usually show nothing useful, and most people are much better inside six weeks. The pain is real, but the back is not “damaged” in the way people fear, and that fear is what leads to the bed rest that slows recovery.
The desi triggers are specific. Sitting cross-legged on the floor for two hours at a majlis and then standing up in one go. Lifting a 20 kg atta bag or a gas cylinder with straight legs and a twist. Sleeping on a mattress that has a valley in the middle from ten years of the same side. Long hours on a motorbike over broken roads. Pregnancy. And, more and more, ten hours a day in a chair over a laptop, which our desk posture reset was written for.
Kamar dard ka gharelu ilaj: the first 48 hours
The plan is simple and slightly counter-intuitive.
- Keep moving. Walk about the house every hour for 5 minutes, even if it’s slow and stiff. Movement pumps fluid through the joints and stops the muscles guarding harder. Avoid the two things that hurt most (usually bending forward and lifting) and do everything else.
- Rest in short doses. Twenty minutes lying on your back with a pillow under the knees, or on your side with a pillow between them, then get up. Not a whole day flat, and never more than one.
- Heat for a tight, spasming back. A hot water bottle wrapped in a towel, or a warm heating pad, on the low back for 15 to 20 minutes, up to 4 times a day. Warm, not scalding; check it on your inner forearm. The winter stiffness post has a warm oil massage that suits the same purpose.
- Cold if it followed a specific injury. A fall or a sharp wrench with swelling: a cloth-wrapped ice pack for 10 to 15 minutes, 3 or 4 times in the first day, then switch to heat. Never ice on bare skin.
- Pain relief by the pack. Paracetamol at the dose on the box, taken regularly for two or three days rather than waiting for the pain to peak, lets you keep moving. Ibuprofen is an option for some adults but not for anyone with kidney disease, stomach ulcers, heart failure, on blood thinners, or in pregnancy, and not on an empty stomach; ask a pharmacist. No muscle-relaxant tablets or injections from a medical store without a doctor.
- Sleep set-up. On your side with a pillow between the knees, or on your back with one under the knees. If the mattress sags, put it on the floor for a week or slide a folded razai under the dip.
Two things are missing from that list on purpose. Massage from a “pehlwan” who cracks your back: no, not in the first days and not by anyone untrained. And an oil that “fixes the disc”: no oil reaches a disc. Warm massage feels good and loosens muscle. That’s its honest job.
Three stretches that are safe on day two
Start these once the sharpest pain has passed, usually day two or three. Gentle, slow, never into sharp pain. Two or three times a day on a mat or a folded razai.
Knee to chest
Lie on your back, knees bent, feet flat. Pull one knee toward your chest with both hands, hold 20 seconds, breathe, lower it. Other side. Five each side. Then both knees together if that’s comfortable.
Pelvic tilt
Same position. Flatten your lower back into the floor by tightening your stomach and tilting your hips up, hold 5 seconds, release. Ten times. It’s small. It should feel like nothing much, and that’s right.
Cat and cow
On hands and knees. Round the back up toward the ceiling like a stretching cat, then let the belly drop and lift the head. Slowly, 10 rounds, moving with the breath. Skip it if being on your hands and knees hurts your wrists or knees.
After a week, add walking as the main exercise: 10 minutes twice a day, building to 30. Walking is the best-tolerated back exercise there is. Swimming if you have access. No sit-ups, no toe-touches, no heavy lifting until you’re pain-free for two weeks.
The old household answer to kamar dard was a warm sarson oil malish with a pinch of ajwain or a few cloves of garlic heated in it, a hot cloth or a heated brick wrapped in a towel on the back, and methi dana in the food. Hakeems classed most back pain as cold and wind in the joints, to be answered with warmth and dryness. The heat part is sound and matches what physiotherapists suggest today. The garlic and methi are food, not treatment. And the traditional “complete bed rest for a week” is the one piece of old advice that has been turned on its head.
Floor sitting, lifting and the mattress
Floor sitting is not bad for the back, but two hours cross-legged without moving is. Shift position every 20 minutes, sit against a wall, put a cushion under the hips so the knees sit lower than the hips, and stand up by rolling to your knees first rather than levering up from a slump. Older people and anyone with a bad back are allowed a chair. Say so at the majlis.
Lifting: get close to the load, bend your knees, keep the back straight, breathe out as you lift, and turn with your feet instead of twisting the spine. Half the sack twice beats the whole sack once. A 20 kg atta bag is a two-person lift for a lot of people, and there’s no shame in it.
Mattresses: medium-firm suits most backs. Rock hard is not better. A mattress with a hollow where you sleep is worse than a firm floor mattress, so rotate it every three months, and replace it when the dip is visible.
Sciatica, and the signs that are not for home care
Sciatica means the nerve that runs down the leg is being irritated, usually by a bulging disc. The pain shoots from the buttock down the back of the thigh and below the knee, often to the foot, sometimes with pins and needles. One leg, typically. It’s miserable, but most sciatica also settles over weeks with the same plan as above, and it needs a doctor if it’s not improving after 2 weeks, if the leg is getting weaker, or if the foot drags.
You have numbness or tingling in the groin, buttocks or inner thighs (the area that would touch a saddle). You cannot pass urine, or cannot feel it coming, or have lost control of bladder or bowels. Your legs, one or both, are weak or numb, or getting weaker. These can mean pressure on the nerves at the base of the spine and the window to treat it is short. See a doctor within a day or two if the pain followed a fall or accident, you are over 50 with a first-ever severe episode, you have a history of cancer, osteoporosis or long steroid use, you have fever or unexplained weight loss, the pain is constant and worse at night or lying down, or it has not improved at all after 6 weeks. In pregnancy, back pain is common, but tell your antenatal doctor about it, and do not take ibuprofen or any pain medicine beyond paracetamol without asking. Do not start or stop any prescribed medicine on your own.
Right now, if your back has just gone
Get off the sofa and walk to the gate and back, slowly. Paracetamol by the pack. Hot water bottle in a towel for 15 minutes while you sit upright. Tonight, a pillow between the knees. Tomorrow, the same, plus the knee-to-chest stretch. Then read the red flags once more.

Should I use heat or cold for kamar dard?
Heat for stiffness and muscle spasm, which is most back pain, 15 to 20 minutes up to 4 times a day. Cold only in the first day after an actual injury with swelling. Both wrapped in cloth, never on bare skin.
Is bed rest good for back pain?
A day at most. Beyond that it stiffens the back, weakens the muscles and slows recovery. Short rests, then get up and walk.
Can floor sitting cause back pain?
Long, unmoving floor sitting can trigger it, especially standing up suddenly afterwards. Change position every 20 minutes and put a cushion under your hips.
Which mattress is best for lower back pain?
Medium-firm for most people. Replace any mattress with a visible dip. Sleeping on a thick mattress on the floor for a week is a cheap test of whether firmer helps you.
Does mustard oil massage help back pain?
Warm massage relaxes tight muscle and feels good, which helps you move. It does not reach or repair a disc, and it should not be forceful in the first days.
When does back pain need a scan?
Usually not in the first 6 weeks unless there are red flags such as numbness in the saddle area, bladder problems, leg weakness, fever, weight loss, trauma or a cancer history. A doctor decides.
How long does sciatica take to go?
Most sciatica improves over 4 to 8 weeks with movement and pain relief. See a doctor if it is not improving after 2 weeks, or sooner if the leg weakens.
