Cluster Headache and Other Rare Severe Headaches: Do Not Wait
A headache that reaches its worst within a minute, a stabbing pain behind one eye that returns at the same hour every night for weeks, a new headache after 50 with a tender scalp and an aching jaw, a burning one-sided pain on the forehead that turns into a rash: these are not headaches to manage with chai and a balm. Each has a name, each has a treatment, and each gets worse the longer it waits. This guide describes the rare severe headaches plainly, tells you what to do in the first hour, and gives you the words to say at the hospital or clinic so the right test happens fast.
The headaches that mean emergency care now
Call 1122 in Pakistan or 112 in India, or have someone drive you to the nearest emergency department, if any of these applies. Do not drive yourself. Do not sleep on it.
Thunderclap headache
From nothing to the worst pain of your life in under a minute, like a blow from inside the skull, during exertion, sex, straining or for no reason at all. Some settle within hours, and that is the trap: a bleed around the brain can present as a “warning leak” that eases, then rebleeds days later. Every thunderclap headache is a scan the same day, even if it has gone by the time you arrive. Say the words “sudden, worst-ever, peaked within a minute”.
Headache with fever and a stiff neck
Fever, a severe all-over headache, a neck that will not bend the chin to the chest, dislike of light, drowsiness or confusion, and sometimes purple spots that do not fade under a pressed glass. Meningitis until proven otherwise, adult or child. Hours matter.
Headache with stroke signs
Weakness or numbness of one arm, leg or side of the face; slurred or lost speech; confusion; loss of vision in one eye or one half of the field; loss of balance. Headache after a head injury
A fall, a motorbike, a cricket ball, a blow in a fight. A headache that worsens over the following hours, with vomiting, drowsiness, confusion, unequal pupils or a fit, is bleeding inside the skull until a scan says otherwise. Over 60, on blood thinners, or a heavy drinker? Bleeds in those groups build slowly over days, so the threshold is lower still.
Headache with a painful red eye and halos
Severe pain in one eye, a red eye, blurred vision with rainbow halos around lights, nausea and vomiting, often starting in dim evening light. Headache with a blood pressure of 180/120 or higher
High BP rarely causes headache; more often the pain raises the reading. The exception is 180/120 or more with symptoms (severe headache, chest pain, breathlessness, confusion, vision change), a hypertensive emergency. In pregnancy after 20 weeks, a headache with swelling, vision changes or a BP of 140/90 or higher is pre-eclampsia until checked, the same day.
Cluster headache: the one-sided eye pain in cycles
Cluster headache is rare, mostly affects men, usually starts between 20 and 40, and is mistaken for years as migraine, sinus or a bad tooth. Once you know the pattern, it is unmistakable.
The pain is always one-sided, in or behind one eye, and boring or stabbing rather than throbbing; people describe a hot poker. It reaches full force within 5 to 10 minutes and lasts 15 minutes to 3 hours, then stops. On the same side the eye waters and reddens, the lid droops, the nostril blocks or runs. Migraine sufferers lie still in the dark; cluster sufferers pace and cannot keep still. Attacks come once to several times a day, often at the same hour and often an hour or two into sleep, for weeks or months (the cluster), then vanish for months or years before returning, often in the same season. Alcohol during a cluster brings an attack within the hour.
Why not wait: ordinary painkillers do almost nothing, because the attack ends before a tablet works. The treatments that do work (high-flow oxygen by mask, a fast-acting injection or nasal spray, a preventive to break the cluster) all need a neurologist and a correct name first. People lose years to the wrong label. Write down a week of timings and ask for a referral.
Cluster or migraine? The differences
- Cluster: 15 minutes to 3 hours; strictly one side, in or behind the eye; watering red eye, droopy lid, blocked nostril on that side; restless, pacing; same time each day for weeks; more often men.
- Migraine: 4 hours to 3 days; one or both sides, throbbing; nausea, light and sound sensitivity; wants to lie still in the dark; irregular timing, with triggers; more often women. Our migraine triggers guide covers that pattern.

Temporal arteritis: the new headache after 50
Giant cell arteritis is inflammation of the arteries of the scalp and head, almost always over 50 and more often after 60. The headache is new, usually at the temple, with a tender scalp (combing hurts, the temple artery feels thick and sore), an ache in the jaw on chewing that eases at rest, tiredness, low fever, weight loss and stiff shoulders on waking. The danger is the eye. Untreated, it can take the sight of one eye suddenly and permanently, then the other, so any blurring, double vision or curtain over the vision with these symptoms is an emergency that day.
Diagnosis is a blood test (ESR and CRP), sometimes an ultrasound or biopsy of the temple artery, and treatment starts as soon as it is suspected because sight is at stake. Say: “new headache after 50, tender temple, jaw pain on chewing”.
Shingles on the face: the burning one-sided pain before the rash
Shingles is the chickenpox virus waking up in a nerve. When it picks the nerve of the forehead and eye, the first sign is a burning or stabbing pain on one side of the forehead and scalp, stopping sharply at the midline, 2 to 4 days before any rash; then blisters appear along the same strip. Blisters on the tip or side of the nose mean the eye is likely involved, and that can scar the cornea. Antiviral treatment works best within 72 hours of the rash, so see a doctor as soon as the first spots appear, and get the eye examined if the nose is involved. No balm, no haldi, no totkay on the blisters.
What to do in the first hour, and what to tell the doctor
The doctor needs a timeline more than a pain score. Do these on the way.
- Note the exact time the headache started and how long it took to reach its worst: under 1 minute, 5 to 10 minutes, or over an hour. This single fact separates a bleed from most other headaches.
- Check the neck: can the chin touch the chest? Check the face in a mirror for a droop, and hold both arms out for 10 seconds to see if one drifts down.
- Take a blood pressure reading if a machine is at hand, sitting, after 5 minutes of rest, and write down the numbers.
- Take a temperature. Above 38 °C with headache and neck stiffness changes everything.
- Look at the eyes: one red and painful, one lid drooping, unequal pupils, any loss of vision when each eye is covered in turn.
- List every medicine taken in the last 24 hours, blood thinners above all, and any fall or head knock in the last 2 weeks.
- Bring someone who saw what happened, because confusion, a fit or slurred speech are things the person with the headache often cannot describe.
At the desk, lead with the pattern, not the pain: “sudden worst-ever headache, peaked within a minute”, or “headache, fever and I cannot bend my neck”, or “new headache after 50 with jaw pain on chewing”. These phrases are what triage staff listen for.
On blood thinners, over 60, a weakened immune system, cancer or HIV, pregnant after 20 weeks or within 6 weeks of delivery, an aneurysm in the family, or a headache pattern that has changed sharply from your usual: a lower threshold applies to all of you, and “it eased with a tablet” is not reassurance. A child with a severe headache, vomiting, a stiff neck, drowsiness, a rash, or a headache that wakes them from sleep is taken to hospital, not watched. Old Unani texts describe “aadha sar dard” carefully and prescribed shade, rest and cooling, which is fair for migraine. They could not see a bleed, an inflamed artery or a rising eye pressure, so tradition offers nothing for this page except one true instinct: a headache unlike any before is a different illness, not a stronger version of the old one. Is a headache with a stiff neck always meningitis?

What does temporal arteritis feel like?

How long after a head injury can a headache be dangerous?
Most bleeds show within 24 to 48 hours, but slow bleeds in older people, heavy drinkers and those on blood thinners can build over weeks. A worsening headache, confusion, drowsiness or vomiting after any head injury needs a scan, however long ago the knock.
Can shingles cause a headache before the rash?
Yes. One-sided burning or stabbing pain on the forehead and scalp, stopping at the midline, comes 2 to 4 days before the blisters. See a doctor as soon as spots appear, urgently if the nose tip or eye is involved.
Do painkillers work for cluster headache?
Ordinary tablets are too slow; the attack is over before they act. Cluster headache has specific fast treatments and preventives that need a doctor’s diagnosis, which is why the correct name matters.
When should an older person worry about a new headache?
Any new headache after 50 deserves a doctor’s visit, urgently with a tender temple, jaw pain on chewing, vision change, weight loss, confusion, weakness or a recent fall.
