Treadmill Test Overview

Here's the short, honest answer, and one tip most people miss.

What the test does

The treadmill test records your heart’s electrical activity while you walk on a speeding, tilting belt. It makes the heart work harder, then watches for any dip that signals reduced blood flow.

Reassuring fitness level

Reaching about ten METs, ten times the energy you use at rest, is generally seen as a reassuring sign of heart fitness.

Bruce protocol basics

Most labs use the Bruce protocol: every three minutes the treadmill speeds up and steeps, turning a brisk walk into a jog uphill. The report notes how many minutes you lasted.

Result meanings

A positive result shows a one-millimeter ST segment dip, often with chest pressure or blood-pressure drop. Negative means no dip, but only if you hit the target heart rate. Inconclusive appears when you stop early due to fatigue or pain.

After a positive

If the test is positive, you’ll meet a cardiologist within days. They may order a CT coronary angiogram, a stress echo, or a nuclear scan. A catheter angiogram is reserved for strong suspicion of a blockage.

Preparation tips

Eat a light snack two to three hours before, avoid coffee, tea, and smoking, and wear loose clothing and good walking shoes. Bring your medication list and ask the doctor which drugs, if any, to pause.

Target heart rate

A clear result usually requires reaching at least eighty-five percent of your predicted maximum heart rate during the test.

The one tip for a clear result

The single most useful tip is to push until you reach at least eighty-five percent of your predicted maximum heart rate, unless symptoms force you to stop. Communicate any chest pressure, dizziness, or breathlessness right away.
