Tongkat Ali and Herbal Men’s Capsules: The Hidden Drug Problem

A “Malaysian herbal” men’s capsule that works within an hour isn’t herbal. Tongkat ali (Eurycoma longifolia, also pasak bumi) is a genuine bitter root with some modest, early evidence on testosterone, but the bigger story is what regulators keep finding inside imported “tongkat ali” capsules, honey sachets and timing pills: undeclared sildenafil, tadalafil and steroids. Those hidden drugs can kill a man on nitrate heart medicine. This guide covers the root honestly, then shows how to spot the risky products and when to see a doctor.

Read this first if you have a heart condition

If you use a nitrate for angina (glyceryl trinitrate spray or tablets under the tongue, isosorbide mononitrate or dinitrate, often sold as Sorbitrate), never take any “herbal” men’s capsule, sachet or honey. A hidden erection drug plus a nitrate can drop blood pressure so far that a person collapses or dies. The same goes for recreational “poppers”. If someone feels faint, has chest pain or collapses after such a product, call an ambulance or go to emergency now, and bring the packet.

What is tongkat ali?

It’s a slender rainforest tree from Malaysia, Indonesia, Thailand and Vietnam. The root is sold. It’s brutally bitter. “Tongkat ali” means “Ali’s walking stick” in Malay, a reference both to the long, straight root and to its reputation for men’s vigour.

It contains bitter compounds called quassinoids plus small proteins and alkaloids. In South Asia you mostly meet it as imported or online capsules, because it isn’t part of the old Unani books at all; it arrived through trade and marketing.

Before you buy

  • Honest evidence: small trials hint at modest testosterone and mood effects in some men, especially those with low levels. Far from settled.
  • Real danger: undeclared drugs in “herbal” men’s products, and heavy metals in poorly made ones.
  • Red flag: any effect within an hour, flushing, headache or blue-tinted vision.
  • Verdict: the root is a maybe; anonymous imported “power” capsules are a skip.

What people say: the famous claims

Online, tongkat ali is “Malaysian ginseng”, a natural testosterone booster, a libido fixer and a stress herb, sold with photos of bodybuilders and words like “royal” and “VIP”.

How it was used

In Malay and Indonesian folk medicine, boiled root water from home-cut chips was drunk after childbirth, for fevers, as a bitter tonic, and by men for strength and desire. Tradition, not evidence.

Our honest take on tongkat ali itself

Depends The root has more human testing than most men’s herbs, but that bar is low.

Plausible, early evidence

A small number of controlled trials, many funded by companies selling standardised extracts, described modest rises in testosterone in men with low or borderline levels, and slight improvements in mood or stress scores. Effects were small, groups were small, and products were specific patented extracts, not whatever’s in a random capsule.

Not proven

It isn’t proven to fix erectile dysfunction, build significant muscle, or raise testosterone in men whose levels are already normal.

False

Nothing herbal works like a prescription erection pill within an hour. If something does, it probably contains one.

The hidden drug problem: what regulators keep finding

For well over a decade, regulators in the US, UK, Europe, Australia, Singapore, Malaysia and the Gulf have issued alert after alert about men’s “herbal” products. Same pattern everywhere.

  • Sildenafil and tadalafil: the active drugs in prescription erection pills, hidden in capsules labelled tongkat ali, ginseng, maca, horny goat weed or simply “herbal”.
  • Analogues: chemical cousins of these drugs that have never been tested in people, used to dodge lab screens.
  • Honey sachets: “royal honey” or “VIP honey” for men, with hidden drugs mixed into the honey.
  • “Timing” pills: imported delay products found to hide dapoxetine or anaesthetics.
  • Steroids: gym “testosterone booster” capsules hiding anabolic steroids or prohormones.
  • Heavy metals: some tongkat ali products in Malaysia were recalled for high mercury or lead.

The hidden amount varies wildly from capsule to capsule, sometimes far above a normal prescription, and because a man doesn’t know he’s taking it, he can’t warn his doctor or an emergency team. That’s the trap.

The biggest danger in a “herbal” men’s capsule is the drug nobody told you about.

Who is most at risk from hidden drugs?

  • Men on nitrate heart medicine (deadly drop in BP).
  • Men with heart disease, recent heart attack or stroke, or very high or very low BP.
  • Men on alpha-blockers for prostate or BP (tamsulosin, doxazosin), which add to the BP drop.
  • Men on HIV medicines or some antifungals and antibiotics, which raise the level of sildenafil in the blood.
  • Men with eye disease, kidney or liver disease, or sickle cell disease.

Steroid-laced products add raised blood sugar, mood swings, acne, shrunken testicles, breast growth and stalled sperm production that can take months to recover.

How to check a product before you take it

  1. Look for the registration. In Pakistan, check the pack for a DRAP enlistment or registration number; our guide on checking DRAP enlistment explains this in general terms. In India, look for an AYUSH licence. No number, foreign script only, or no manufacturer address means skip.
  2. Read the promises. “Works in 30 minutes”, “lasts 72 hours”, “100% natural, no side effects” and pictures of men in suits or bodybuilders are classic signs of a spiked product.
  3. Check the format. Single loose capsules, unbranded blister strips, honey sachets and products sold only through Facebook, WhatsApp or roadside stalls are highest risk.
  4. Search the name plus “FDA warning” or “tainted”. Many spiked products are named publicly, though a clean search proves nothing.
  5. Know the side-effect signs. Headache, flushed face, blocked nose, indigestion, dizziness or blue-tinged vision within an hour or two point to a hidden erection drug.
  6. Keep the packet. If you react, a doctor needs to see it, and you can report it to DRAP or your local regulator.

We don’t give amounts for tongkat ali or any men’s product. The pack or a qualified practitioner sets that, after checking your heart and medicines.

Which tongkat ali myths do sellers rely on?

Myth: herbal means no side effects

Even genuine tongkat ali can cause sleeplessness, restlessness, irritability and a faster heartbeat in some men, and it may interact with blood pressure, diabetes and immune-suppressing medicines. Then add whatever else is in the capsule.

Myth: a booster is safer than seeing a doctor

Erection problems are often the first visible sign of narrowed arteries, years before a heart attack, and they’re common with diabetes, high BP, high cholesterol, smoking, obesity, depression and some medicines, all of which a doctor can find and manage with treatment that has actually been tested. Read mardana kamzori as a warning sign and, if you suspect low testosterone, our guide to the testosterone test report. For a local herb with similar claims, see safed musli.

Red flags: when to see a doctor or go to emergency

Emergency now: chest pain, fainting, sudden severe headache, sudden loss of vision or hearing, or an erection lasting more than 4 hours after any product. Don’t wait that one out; it can cause permanent damage.

See a urologist or andrologist soon: erection problems lasting more than a few weeks, low desire with tiredness or breast growth, a testicular lump or pain, blood in semen or urine, or a curve in the penis.

Not for anyone under 18, nor in pregnancy or breastfeeding. Men with heart disease, diabetes, liver or kidney disease, or on regular medicine should talk to their doctor before any men’s product, even a “pure” root. Never stop or change prescribed medicine to try one. More honest explainers sit in Unani & Herbal, Decoded.

Did you know?

  • Tongkat ali root is so bitter that it’s often sold mixed into instant coffee in Malaysia to make it drinkable.
  • Sildenafil was first tested as a chest pain medicine. Its effect on erections was noticed during those trials, which is exactly why it clashes so badly with nitrate heart drugs.

Tongkat ali and men’s capsule questions

What are tongkat ali benefits?

Modest, at best. Small controlled trials hint at slight testosterone, mood and stress effects, mainly in men with low or borderline levels, but it isn’t proven for erectile dysfunction or muscle building and product quality varies enormously.

Does tongkat ali increase testosterone?

Possibly a little, in men whose levels are low. Men with normal levels are unlikely to notice much, and a morning blood test tells you where you stand before you spend anything.

Is tongkat ali safe?

The root, maybe; the capsules, often not. In trials the root seemed fairly well tolerated short term, but imported capsules carry the risk of hidden drugs and heavy metals, so anyone with heart disease, diabetes or regular medicine should ask a doctor first.

Why do some herbal men’s capsules work so fast?

Hidden drugs. Many contain sildenafil or tadalafil, the active ingredients of prescription erection pills, and regulators worldwide keep finding them, so a fast, strong effect is a warning sign rather than a recommendation.

Can I take a herbal men’s capsule with heart medicine?

Not with nitrates. If you use isosorbide or glyceryl trinitrate, a hidden erection drug can cause a fatal fall in blood pressure; with any other heart medicine, show the pack to your cardiologist first.

What is royal honey or VIP honey for men?

Spiked honey, often. These sachets are sold as natural boosters, yet regulators have repeatedly found undeclared sildenafil or tadalafil mixed in, so treat them like an unlabelled prescription drug and leave them alone.

Are Malaysian or Chinese timing pills safe?

Many aren’t. Imported timing tablets and sprays have been found to hide dapoxetine, anaesthetics or erection drugs, and since early ejaculation is common and treatable, a doctor is the safer route.

How do I know if a capsule has a hidden drug?

You can’t, at home. Warning signs include no registration number, huge promises, an effect within an hour, headache, flushing, a blocked nose or blue-tinted vision.

Is tongkat ali registered with DRAP?

Some products may be; many sold online aren’t. Look for an enlistment or registration number on the pack and check it on DRAP’s official site, and if there’s no number, don’t buy.

Tongkat ali ke side effects kya hain?

Sleeplessness, restlessness, irritability and a faster heartbeat are reported with the root itself. Spiked products add headaches, flushing, low BP, prolonged erections and, when steroids are hidden inside, raised sugar and shrunken testicles.

Can tongkat ali help with fertility?

Not proven. Worse, steroid-laced boosters can shut down sperm production, so get a semen analysis and see a fertility doctor if you’ve been trying for a year.

Is tongkat ali good for gym and muscle?

Barely. Training, protein and sleep matter far more, and gym “test boosters” are a favourite hiding place for steroids with serious long-term harm.

What should I do if I feel unwell after a men’s capsule?

Stop it now. Go to emergency for chest pain, fainting, sudden vision loss or an erection lasting over 4 hours, and take the packet so the doctors know what you swallowed.

Can weak erections be a sign of heart disease?

Yes. The small arteries in the penis often narrow before those in the heart, so erection trouble can appear years before a heart attack, and it’s also common with diabetes, which is why a check-up matters.

Can I take tongkat ali with diabetes medicine?

Ask your doctor first. It may affect blood sugar, hidden steroids in some products push sugar up sharply, and diabetes medicine should never be changed on your own.

Is tongkat ali the same as ginseng?

No. Tongkat ali is a Southeast Asian tree root and ginseng a different plant from Korea, China and North America, though both are marketed for vigour and both turn up in spiked products.

Where can I report a fake herbal product?

To your regulator. In Pakistan that’s DRAP; in India, the AYUSH ministry or the state drug controller. Keep the packet, batch number and purchase details.
