Strep Test Results: Rapid Throat Swab vs Culture Explained
A positive strep test means group A streptococcus was found on the throat swab, and the doctor will usually treat it to prevent complications. A negative rapid strep test in an adult usually settles the question; in a child over 3, a negative rapid test is often double-checked with a throat culture because the rapid test can miss some cases.
At a glance
- Rapid strep test: antigen swab, result in about 10 to 20 minutes; quick but can miss infections.
- Throat culture: the gold standard; result in 24 to 48 hours.
- Most sore throats: viral, and antibiotics do nothing for them.
- Viral clues: cough, runny nose, hoarse voice, mouth ulcers, red watery eyes.
- ASO is different: a later blood test, not a test for today’s sore throat.
Does every sore throat need a strep test?
No. Most sore throats are caused by viruses, and they settle on their own. CDC guidance says that when someone has clear viral signs (a cough, runny nose, hoarseness, mouth ulcers or conjunctivitis) testing for strep usually isn’t needed, because strep throat rarely comes with those.
Strep tends to look different: sudden sore throat, painful swallowing, fever, swollen tender glands at the front of the neck, sometimes white patches on red tonsils, and often a headache or tummy ache in children, but no cough or cold. Doctors use scoring checklists built from these clues to decide who to swab. They’re tools for the clinician rather than a home diagnosis, because exam alone can’t reliably separate viral from strep.
Rapid strep test or throat culture: which is accurate?
| Test | Result time | Strength | Weakness |
|---|---|---|---|
| Rapid antigen (RADT) | 10 to 20 min | Positive is reliable | Can miss some infections |
| Throat culture | 24 to 48 h | Most accurate | Slow; needs a follow-up call |
| ASO titre (blood) | Same day to a few days | Shows a past strep infection | Rises weeks later; useless for today |
The reason children get the backup culture is rheumatic fever. Untreated strep in school-age children can, rarely, lead to rheumatic fever, which can damage heart valves. Treating confirmed strep lowers that risk. In adults rheumatic fever is very rare, so a negative rapid test is generally accepted.
What about children under 3?
Strep sore throat is uncommon in toddlers. In this age group strep more often shows up as a runny nose with thick discharge, poor appetite, irritability and a low fever, so doctors test less often. Under 6 months, any fever or poor feeding is a reason to see a doctor directly rather than think about strep at all.
- Under 6 months and 6 to 12 months: fever, poor feeding or unusual crying means a doctor, not a home test.
- 1 to 3 years: strep throat is uncommon; viral causes dominate.
- 3 to 6 and over 6: peak age for strep; a negative rapid test is commonly followed by culture.
The swab answers today’s question. The ASO blood test answers a question from weeks ago.
Strep test negative but sore throat: now what?
For most adults and teens, a negative test plus a sore throat means a viral infection. Rest, fluids, warm drinks and gargles help comfort; our salt water and haldi gargle guide gives the mix, and the tonsils home care article covers the rest. For a child over 3, ask whether a culture was sent and how you’ll hear the result, and keep the clinic’s number.
Many families are then handed an ASO test “to check for strep”. ASO measures antibodies the body makes after a strep infection, which take weeks to rise, so it can’t diagnose a sore throat that started yesterday. Our ASO titre guide explains when it’s genuinely useful.
Make the swab count
Ask the nurse whether they want you to skip lozenges, throat sprays and mouthwash beforehand, and simply leave them for that morning to be safe. For a child, sit them on your lap facing forward with their head against your chest and hold their forehead gently; a firm, quick swab of both tonsils and the back of the throat is over in seconds and less likely to need repeating.
- Note when the sore throat began and whether there is cough or a runny nose.
- Take the temperature with a thermometer and write it down.
- At the clinic, ask: “Rapid test or culture, and will a negative be cultured?”
- If antibiotics are prescribed, finish the course as directed; never use leftover tablets from a previous illness.
- Fever medicine for children goes by weight, from the pack or a pharmacist.
Fever care at home, including sponging and when it’s enough, is in our fever home care guide.
Go urgently for drooling or being unable to swallow saliva, a muffled “hot potato” voice, trouble breathing or noisy breathing, inability to open the mouth fully, swelling on one side of the neck or above one tonsil (possible quinsy), a stiff neck, a rash that does not fade under a pressed glass, or a child who is drowsy, floppy or refusing all fluids. Any fever in a baby under 3 months needs a doctor the same day. Weeks after a strep infection, report joint pain and swelling, breathlessness, or dark tea-coloured urine and puffy eyes, as these can be late complications.
Strep test questions people ask
What does a positive strep throat test mean?
It means group A streptococcus was found on the swab. The doctor will usually prescribe a course of antibiotics to shorten the illness and prevent complications.
How accurate is the rapid strep test?
A positive result is reliable, but the test can miss some true infections. That is why children over 3 with a negative rapid test often get a throat culture too.
Strep test negative but sore throat, why?
Most sore throats are viral, so a negative result usually fits. In a child the rapid test can miss strep, so ask whether a culture was sent.
What is a throat swab culture report?
It shows whether group A strep or other significant bacteria grew from the swab over 24 to 48 hours. It is more accurate than the rapid test.
Can a child have a throat swab?
Yes, it is quick and safe, though it may cause brief gagging. Holding the child on your lap with their head steadied helps.
Does strep need a test or can the doctor tell by looking?
Looking alone cannot reliably separate strep from a viral throat. A swab is needed to confirm it, unless clear viral signs make testing unnecessary.
How much does a group A strep test cost?
Prices vary between clinics and labs, and culture usually costs more than a rapid test. Ask the lab for the current rate.
Is ASO the same as a strep test?
No. ASO is a blood test for antibodies that rise weeks after strep, so it cannot diagnose a current sore throat.
Can adults get strep throat?
Yes, though it is more common in school-age children. In adults a negative rapid test is usually accepted without a culture.
