Seborrheic Dermatitis on the Face: Flaky Eyebrows and Nose Folds

Greasy, yellowish flakes in the eyebrows, red scaly skin in the folds beside the nose, and “dandruff” in the beard are the classic signs of seborrheic dermatitis on the face. It flares in winter, it comes back, and it is controllable. The routine that works is simple: an antifungal wash left on long enough, no oiling, and a light moisturiser.

Quick facts

  • Where: eyebrows, between the brows, sides of the nose, beard and moustache, eyelid edges, behind the ears, hairline.
  • Looks like: mild redness with greasy white or yellow scales; on brown skin it may look pinkish, grey or slightly lighter than the surrounding skin.
  • Linked to: oily areas and Malassezia, a yeast that lives on everyone’s skin.
  • Worse with: cold dry weather, stress, poor sleep, heavy oils and creams.
  • Nature: long-term and comes and goes; control, not a one-time fix.

What is seborrheic dermatitis on the face?

It is the same condition as dandruff, just on facial skin. The oil glands in the T-zone, brows and beard are busy, and a normal skin yeast called Malassezia feeds on that oil. In some people the skin reacts to the yeast and its by-products with inflammation and fast shedding, which shows up as flakes on a red base.

It isn’t caused by poor hygiene and it isn’t contagious. It tends to run in families, flares with stress and cold weather, and is more common in people with oily skin and in some neurological conditions such as Parkinson’s.

The scalp side of things is already covered in our dandruff home remedies and ketoconazole and zinc shampoo guides, so this article stays on the face and beard.

Flaky skin around the nose and eyebrows: is it this, or something else?

Plain dry skin flakes are fine, white and not greasy, and they improve with moisturiser. Seborrheic flakes are larger, a bit greasy, sit on pink or red skin and keep returning to the same spots. Ringworm (daad) is usually a ring with a clearer centre and an active edge, often on one side; see daad and steroid creams. Rosacea, eczema, psoriasis and perioral dermatitis can all look similar, so a doctor’s look is worth it if things don’t settle.

The oil you rub in to soften the flakes is the food the yeast was waiting for.

Why oiling the eyebrows and beard makes it worse

Many families oil brows and beards in winter: coconut, mustard, olive or beard oils. Malassezia uses fatty oils as food, and heavy oils also trap scale. Flakes may look smoother for an hour, then return thicker. Stop oiling the affected areas for at least a month and see the difference.

A face routine for seborrheic dermatitis

  1. Medicated wash, 2 to 3 times a week. Use an antifungal wash or shampoo your doctor or pharmacist recommends (ketoconazole, zinc pyrithione or selenium types). Lather it on the brows, nose folds and beard.
  2. Leave it on for 3 to 5 minutes. Count it out or use a phone timer. A quick rinse does very little because the ingredient needs contact time. Keep it out of the eyes.
  3. Rinse thoroughly with lukewarm water, not hot.
  4. On other days, wash with a gentle, non-soap cleanser.
  5. Moisturise lightly with a non-greasy, fragrance-free cream or gel.
  6. Once calm, keep the medicated wash going once a week to hold it off.

Beard care

Wash the beard with the medicated shampoo, massage to the skin underneath, leave it on for the full time and rinse well. Dry thoroughly. Swap heavy beard oils for a light, non-oily conditioner. Trimming shorter during a flare makes it easier to reach the skin. Some men notice it clears completely when they shave.

The shower timer trick

Apply the medicated wash to your face and beard as the first thing in the shower, then wash your hair and body. By the time you are done, the 3 to 5 minutes have passed and you rinse it all off at the end. No standing around, no skipped contact time.

Ketoconazole on the face: what to know

Ketoconazole washes are widely used on the face for this condition, and ketoconazole creams exist too. Use them as your doctor or the pack directs, keep them away from the eyes, and stop if you get burning, swelling or a rash. If a product stings badly, ask the pharmacist about a gentler option.

The steroid cream caution

Mild steroid creams are sometimes prescribed for short bursts to calm a red flare. The problem is when they are bought over the counter and used for months. On the face, long-term steroids can thin the skin, cause stretch marks, visible vessels, acne-like bumps and a rash around the mouth (perioral dermatitis), and flares often rebound when they stop. Steroid-containing “fairness” and mixed creams sold in markets are a big source of this. Use a steroid on your face only if a doctor prescribed it, for the time they said.

When to see a doctor

See a doctor if the rash doesn’t improve after four weeks of a good routine, spreads, becomes very red, painful, weeps, crusts or drains pus, or affects the eyelids and eyes. Get checked sooner if it appears suddenly and severely, as it can occasionally go with other health problems. A baby with scaly patches (cradle cap) needs gentle care, not adult medicated washes, so ask a doctor. Do not use steroid creams on the face long term, and don’t stop a cream a doctor prescribed without asking them how to taper.

Seborrheic dermatitis questions

Why is the skin around my nose flaky and red?

Seborrheic dermatitis is a common reason: greasy scale on red skin in the nose folds. Dry skin, rosacea and eczema can look similar, so see a doctor if it persists.

Why do I have dandruff in my eyebrows?

Eyebrow flakes are usually seborrheic dermatitis, the same process as scalp dandruff. An antifungal wash left on for a few minutes and no oiling usually helps.

What is the best treatment for seborrheic dermatitis on the face?

Commonly an antifungal wash such as ketoconazole used a few times a week, a light moisturiser and avoiding heavy oils. Doctors may add short-term prescription creams for flares.

How do I get rid of beard dandruff?

Wash the beard with a medicated shampoo, leave it for 3 to 5 minutes, rinse well and dry. Replace heavy beard oils with a light conditioner.

Why is seborrheic dermatitis worse in winter?

Cold dry air, indoor heating, hot showers and less sun all seem to make flares more likely. Many people also oil their skin more in winter.

Can I use ketoconazole shampoo on my face?

Many doctors recommend it for facial seborrheic dermatitis. Keep it out of your eyes, leave it on briefly and rinse well; follow your doctor’s or the pack’s instructions.

Should I put coconut oil on seborrheic dermatitis?

Usually no. Fatty oils can feed the yeast involved and trap flakes. A light, non-oily moisturiser is a better choice.

Is seborrheic dermatitis permanent?

It tends to come and go for years, but regular care keeps it well controlled. Many people only notice it in winter or stressful periods.

Can stress cause seborrheic dermatitis flares?

Stress and poor sleep are well-known flare triggers. Exam season and busy work weeks often line up with flares.

Is seborrheic dermatitis contagious?

No. It comes from your own skin’s reaction to a normal yeast and cannot pass to others.
