Prolactin Test Report: High Level, Stress and Missed Periods

The short answer: a mildly high prolactin is very often caused by stress, a rushed sample, a medicine or an underactive thyroid, so doctors usually repeat it calmly before worrying, while a clearly high value with missed periods, milky discharge, headaches or vision changes needs a proper work-up.

Quick facts

  • What it is: the milk-making hormone from the pituitary, a pea-sized gland under the brain.
  • Units: ng/mL or mIU/L (1 ng/mL is roughly 21 mIU/L).
  • Common trick: stress, sleep, exercise and even the needle can push it up a little.
  • Red flags: new headaches, blurred or narrowed side vision, milky discharge without breastfeeding.

Periods stop for three months, and the pregnancy test keeps coming back negative no matter how many times it’s repeated. The doctor orders a hormone panel, and one line comes back with an H next to it: prolactin. Cue a late-night search, a lot of panic about tumours, and a WhatsApp forward from a cousin who once knew someone with the same number. Breathe. Usually it’s mild. Often it’s temporary. This Lab Reports Decoded guide shows how doctors think it through.

What prolactin does, in plain words

Prolactin’s main job? Milk. After birth, it rises so the breasts produce milk, and it stays up while a mother breastfeeds. Here’s the clever part. High prolactin pauses ovulation. Nature’s spacing trick.

Trouble starts without a baby. The pause button gets pressed anyway. Periods become irregular or stop, ovulation fails, and some women notice a milky fluid from the nipples when they press or even without touching, which understandably frightens them. Men aren’t spared. In men, high prolactin can lower testosterone, leading to low sex drive, erection trouble and sometimes a slow slide in energy that gets blamed on work or age for months before anyone orders the test.

Reading the report columns

A prolactin report is usually one line: test name, your value, unit, and a reference interval. Typical lab ranges, which vary by lab; use the range printed on your report. Many labs give an upper limit for non-pregnant women somewhere around 25 to 30 ng/mL (about 500 to 630 mIU/L) and a lower one for men.

Some reports include a line called macroprolactin. That’s a large, clumped form of prolactin that shows up on the test but barely works in the body. Often harmless. Did you know? Many labs only run this check when the doctor asks, so a surprising number of women chase a result that was mostly harmless big prolactin.

Why is prolactin high?

Everyday reasons

  • Stress and the sample itself: anxiety, a difficult blood draw, poor sleep or a hard workout can nudge it up.
  • Pregnancy and breastfeeding: the most natural cause of all, and always ruled out first.
  • Nipple stimulation: including tight clothing or self-examination just before the test.
  • Underactive thyroid: the signal that pushes the thyroid also nudges prolactin. Our TSH report guide covers that test.

Medicine reasons

Several common medicines raise prolactin. These include some drugs for nausea and acidity (metoclopramide, domperidone), many antipsychotics, some antidepressants, and certain BP medicines. Please don’t stop any of them. Tell the doctor exactly what you take.

Medical reasons

Kidney or liver disease, PCOS (usually only a mild rise), chest wall injury or shingles, and a small benign growth of the pituitary called a prolactinoma. Most prolactinomas are tiny. Not cancer. Size guides the plan.

A mildly high prolactin is repeated before it’s feared.

What a low prolactin means

Low levels are rare. They usually don’t matter outside breastfeeding. After a very difficult delivery with heavy blood loss, a low prolactin with no milk can hint at pituitary injury (Sheehan syndrome), which a doctor checks with other hormone tests.

How to prepare for the prolactin test

  1. Book the sample for the morning, ideally 2 to 3 hours after waking, not straight out of bed.
  2. Sleep normally the night before; skip hard exercise that morning and the day before.
  3. Avoid breast or nipple stimulation for 24 hours, and wear a comfortable bra.
  4. Arrive early and sit quietly for 15 to 20 minutes before the draw, phone down if you can, because a rushed arrival after hunting for parking is exactly the kind of stress that nudges the number.
  5. Carry a list of all medicines, including acidity and nausea tablets, and share it with the doctor rather than stopping any.
  6. Rule out pregnancy.

Some labs take two samples 20 to 30 minutes apart through one cannula to rule out a needle-stress rise. If yours does, the lower value usually counts. Smart.

Myth: high prolactin always means a brain tumour

No. Stress, medicines, thyroid trouble and macroprolactin explain a large share of high readings, which is why a good doctor goes through that list patiently before ordering any scan of the brain. Even when a pituitary growth is found, it’s almost always a small benign one that a specialist can manage without surgery in most cases, and many women go on to have normal periods and pregnancies afterwards. Fear delays the repeat test. Delay never helps. If periods have also gone irregular, our irregular periods checklist shows the other tests doctors pair with it.

When to see a doctor today

Get same-day care for a new, severe or worsening headache with high prolactin, any blurred, double or narrowed side vision (bumping into door frames), or a sudden thunderclap headache with vomiting or vision loss, which is an emergency. Milky discharge without breastfeeding, periods missing 3 months or more, or bloody nipple discharge (a separate breast check is needed) all deserve a prompt appointment. Interpretation belongs to your doctor, and any medicine change is theirs to make.

Prolactin report: common questions

What is the normal prolactin level in females?

Many labs set the upper limit for non-pregnant women at around 25 to 30 ng/mL; ranges differ between labs and units, so compare with the reference printed on your report.

Can stress increase prolactin?

Yes, emotional stress, a painful blood draw, poor sleep and hard exercise can raise it a little. Hence the repeat.

Can high prolactin cause missed periods?

Yes. High prolactin pauses ovulation, so periods may become irregular or stop. Pregnancy and thyroid problems are checked first.

What time is best for a prolactin blood test?

Morning, about 2 to 3 hours after waking, after sitting quietly for a few minutes; very early samples and post-workout samples can read higher.

Is fasting needed for the prolactin test?

Usually not, though some labs prefer it. Follow your lab’s slip.

Why is there milky discharge from the breast without pregnancy?

High prolactin is one common reason, along with thyroid problems and some medicines; it needs a doctor’s visit, and bloody or one-sided discharge needs a breast check.

Can high prolactin stop a woman from getting pregnant?

It can, because it blocks ovulation. Once the cause is found and managed by a doctor, ovulation often returns on its own schedule, and many couples who had been trying for a long time find that the missing piece was this one hormone all along. Our guide to infertility causes in women covers the rest.

What is macroprolactin?

A large, clumped form of prolactin that the test counts but the body barely uses; if most of your prolactin is this form, the high reading may not need treatment.

Do headaches with high prolactin mean something serious?

Not always, but new or worsening headaches with high prolactin deserve prompt review, and any vision change means seeing a doctor the same day.

Can men have high prolactin?

Yes, it may cause low sex drive, erection problems, breast swelling and low energy. Same checks apply.

Can PCOS cause high prolactin?

PCOS can bring a mild rise in some women. A clearly high value still needs other causes ruled out rather than being blamed on PCOS.
