ALP and GGT Test Report: Liver or Bone? How to Tell

ALP and GGT are two liver-panel lines that look alike and mean different things. ALP is made by both the liver and the bones, while GGT comes almost only from the liver and bile ducts, so a doctor reads them as a pair to decide whether a high ALP is a liver story or a bone story.

The short version

  • ALP high, GGT normal: often points away from the liver, toward bone or a normal life stage.
  • ALP high, GGT high: more likely the liver or bile ducts.
  • GGT high alone: common with alcohol, fatty liver and some medicines, and not a diagnosis on its own.
  • Normal for some people: growing teenagers and pregnant women run a higher ALP.
  • Not covered here: SGPT and SGOT, which have their own guide: how to read a high SGPT in an LFT report.

What do ALP and GGT actually measure?

Both are enzymes, which are tiny working proteins that live inside cells. A blood test does not measure how well an organ works. It measures how much of the enzyme has leaked from cells into the blood. More leak, higher number.

ALP (alkaline phosphatase) sits in the highest amounts in the liver, the bile ducts and the bones. GGT (gamma-glutamyl transferase) is mostly a liver and bile duct enzyme. Think of ALP as a smoke alarm in a building with two floors, liver and bone. It rings, but it does not say which floor. GGT is a detector on the liver floor only.

What are typical ALP and GGT ranges?

These are typical adult ranges, which vary by lab and method. Use the range printed on your own report, because the unit and the cut-off differ between labs.

TestTypical adult rangeNote
ALPabout 40 to 130 U/LHigher in children, teens, pregnancy
GGT, menabout 10 to 60 U/LLab cut-offs differ
GGT, womenabout 5 to 40 U/LUsually lower than men

A report that says “ALP 300” may therefore be perfectly ordinary in a ten-year-old and worth a closer look in a 45-year-old man.

Why is my ALP high but GGT normal?

This is the combination that confuses people most, and it is also the most useful clue. Because only ALP rises with bone activity, a high ALP with a normal GGT is more likely to come from bone than from the liver. Growing bones in children and teenagers make a lot of ALP. Healing bones after a fracture can do the same. Low vitamin D with weak bone mineral can lift it as well, which is why doctors often check vitamin D next. If your report shows a low level, see what a vitamin D level of 12 means.

Pregnancy is another ordinary reason, because the placenta also makes ALP. This is why the lab note about age and pregnancy matters.

Insider tip: read the pair, not the single line

Before worrying about a high ALP, look one line down for GGT. If GGT is normal, say so to your doctor in one sentence: “ALP is high but GGT is normal, could this be bone or my age?” It saves a visit’s worth of guessing. If your doctor wants to be sure, an ALP isoenzyme test can show which tissue is making the enzyme, though not every lab offers it.

What does it mean when ALP and GGT are both high?

When both rise together, the liver or the bile ducts are the more likely source. Bile ducts are the thin pipes that carry bile from the liver to the gut. A blockage, inflammation or swelling there pushes both enzymes up. Hepatitis and cirrhosis can also do it. The test cannot name the cause, so the next step is usually an ultrasound and the rest of the liver panel, including bilirubin.

If your ultrasound mentions fat in the liver, our guide to fatty liver grades 1, 2 and 3 explains what the grades mean.

What raises GGT on its own?

GGT is a sensitive enzyme. It reacts to alcohol, fatty liver, diabetes and some medicines, so a mildly raised value is common and often not alarming by itself. Medicines are only a category here: certain long-term medicines for seizures, and some others, can nudge GGT up. Never stop or change a medicine because of one number. Take the report and your medicine list to the doctor who prescribed them.

Heavy drinking is the best-known cause, and most people with alcohol use disorder have a high GGT. A normal GGT does not prove someone drinks nothing, and a high one does not prove they drink.

Can ALP be low?

Yes, though it is less common. Poor nutrition, low zinc or protein intake, thyroid problems and a few rare inherited conditions can lower it. A single slightly low value with no symptoms usually just gets repeated.

When to see a doctor

This guide explains a report. It cannot say what your numbers mean for you. See a doctor soon if ALP or GGT is high and you also have yellow eyes or skin, dark urine, pale stools, constant itching, upper right belly pain, or unexplained weight loss. Go the same day for severe belly pain with fever or vomiting. Bone pain that wakes you at night with a high ALP also deserves a prompt visit. Pregnant women should ask their own doctor before reading anything into an ALP value.

How should you prepare for the test?

  • Many labs ask for 8 to 12 hours of fasting when ALP is part of a full panel, because a fatty meal can raise it slightly. Follow the lab’s instruction.
  • Skip alcohol for a couple of days before if your doctor is checking GGT, since it can raise the number.
  • Tell the lab about every medicine you take, including herbal products, so the doctor can read the result fairly.

Frequently asked questions about ALP and GGT

What are the most common causes of high ALP?

Common causes are liver or bile duct problems, bone growth or bone disease, and normal life stages such as childhood, the teen years and pregnancy. The ALP number alone cannot tell which, so doctors look at GGT and the rest of the panel.

What is the normal range for alkaline phosphatase?

A typical adult range is roughly 40 to 130 U/L, but labs differ. Children and teens normally run higher. Always compare your number with the range printed on your own report.

What does a high GGT mean?

It means the liver or bile ducts may be under strain, but it does not name a cause. Alcohol, fatty liver, diabetes and certain medicines are common reasons, and a doctor decides what to check next.

Is ALP 300 normal?

It can be normal in a growing child or teenager, and it can be higher than expected in an adult. Age, pregnancy and the lab’s own range decide, so ask the doctor with the full report in hand.

What is the GGT normal range for men and women?

Men usually have a higher normal range than women, roughly 10 to 60 U/L for men and 5 to 40 U/L for women in many labs. Your lab’s printed range is the one to follow.

Why is alkaline phosphatase high in children?

Growing bones make a lot of ALP, so children and teenagers normally show higher values. That is why paediatric reports carry a separate range.

What does it mean if ALP and GGT are both high?

It points more toward the liver or bile ducts than toward bone. The next steps are usually an ultrasound and a fuller liver panel, decided by your doctor.

Can low vitamin D cause a high ALP?

It can, because weak bone mineral makes the bones release more ALP. Doctors often check vitamin D when ALP is high and GGT is normal. Treatment, if any, is the doctor’s decision.

Can fatty liver raise GGT?

Yes. Fatty liver is one of the more common reasons for a raised GGT, often with a mildly raised SGPT as well. An ultrasound confirms the fat.

Do I need to fast for an ALP test?

Often yes if it is part of a full panel, usually 8 to 12 hours, since a heavy meal can nudge it up. Follow what your lab tells you when you book.
