ADHD Symptoms in a Child: Naughty, Bored or Something More?

ADHD symptoms in child behaviour go beyond ordinary naughtiness: they last for months, show up in more than one place (home and school), and get in the way of learning, friendships or family life. A lively, bored or tired child can look similar, so the first step is a calm look at sleep, hearing and vision, then an assessment by a child specialist, never a label from relatives.

Before you worry

  • Three presentations: mainly inattentive, mainly hyperactive and impulsive, or combined.
  • Age: signs usually begin before 12; under 5, high energy and distraction are often normal.
  • The test: not a blood test or scan; a specialist assessment using reports from home and school.
  • Check first: sleep and snoring, hearing, vision, learning difficulties, anxiety.
  • Who assesses: a paediatrician, child psychiatrist or child psychologist.

What do ADHD symptoms in a child look like?

The US CDC groups the signs into three presentations, and a child’s picture can change as they grow.

Mainly inattentive

Daydreaming, losing pencils and water bottles daily, careless mistakes, trouble finishing homework or following multi-step instructions, seeming not to listen. These children are often quiet and get missed, especially girls.

Mainly hyperactive and impulsive

Fidgeting, squirming, leaving the seat, talking a lot, interrupting, grabbing, trouble waiting for a turn, running and climbing at the wrong times. These children get noticed, and often scolded.

Combined

Both sets of signs, roughly equally.

Every child does some of these. What matters is how much, how long, and whether it causes real difficulty at school, at home or with friends, compared with other children of the same age.

Age bands: what is normal and what is not

  • Under 3 years: constant movement and short attention are normal. ADHD isn’t assessed at this age; speech, hearing and development checks are.
  • 3 to 6 years: the NHS notes many children under 5 are easily distracted and full of energy without having ADHD. Specialists are cautious here.
  • Over 6: school demands make difficulties clearer, and assessment becomes more reliable.
  • Teenagers: hyperactivity often settles into restlessness, disorganisation and risk-taking.

Is my child ADHD or just naughty?

“Naughty” usually means a child can behave but chooses not to, often in specific situations. With ADHD, the child often wants to do well and still can’t sit, wait or finish, across many situations, day after day. They’re frequently as frustrated as their parents.

Other things can look just like ADHD. The CDC lists sleep disorders, anxiety, depression and learning disabilities among them. Add hearing problems, poor eyesight, and big family stress. That’s why the first checks matter.

Rule out the tired child first

For two weeks, note bedtime, wake time and whether your child snores, breathes through the mouth or wakes often. A child who sleeps late on screens or snores loudly can look restless and unfocused by day. Get a simple hearing check (our whisper test guide is a starting point) and an eye test. Bring all of it to the appointment.

ADHD test for kids in Pakistan: how assessment works

  1. Start with your family doctor or paediatrician, who checks growth, hearing, vision and sleep.
  2. Ask the class teacher for a written note describing your child’s attention, behaviour and work, with examples. Specialists ask about school, and the NHS describes them speaking to teachers.
  3. Referral to a child psychiatrist, developmental paediatrician or child psychologist, often at a teaching hospital’s child and adolescent psychiatry or psychology department.
  4. Assessment: interviews with parents and child, rating scales from parents and teachers, observation, and history from early childhood.
  5. Plan: parent training, school support, behaviour strategies, and in some cases medicine prescribed and monitored by the specialist.
A diagnosis is a door to support, not a stamp on your child.

ADHD in girls

Girls more often show the inattentive picture: quiet, dreamy, disorganised, “lazy” in report cards. They’re less disruptive, so they’re referred later or not at all, and some carry years of being called careless. If your daughter is bright but can’t finish work or keep track of things, that’s worth raising.

ADHD diet myths and screen time

Sugar doesn’t cause ADHD; our sugar and hyperactivity myth check covers why. Elimination diets, special oils and “brain powders” sold online aren’t a substitute for assessment. Screens don’t cause ADHD either, but long evening screen time can wreck sleep and make attention worse for any child. Our screen time by age guide gives practical limits.

Dealing with stigma at home

Relatives may say the child is spoiled or the mother is too soft. A calm script helps: “We’re getting him checked by a doctor, the same way we’d check his eyes.” Keep the child out of these conversations, and praise effort out loud, because children with attention difficulties hear “no” far more than other kids.

When to get help sooner

See a doctor promptly if your child’s behaviour changed suddenly, if they stop speaking or lose skills, have fits or staring spells, talk of hurting themselves, are harming others, or are being badly bullied. Loud snoring with pauses in breathing at night also needs a doctor. Don’t give a child any medicine, herbal syrup or “calming” product for behaviour without a specialist’s advice, and if a specialist prescribes medicine, changes are made only with them.

Parents ask about ADHD

What are the first signs of ADHD in a child?

Often trouble sitting still, constant interrupting, or losing focus and things far more than other children the same age, lasting months and showing at home and school.

Is there an ADHD test for kids in Pakistan?

There’s no blood test. A child psychiatrist, developmental paediatrician or psychologist assesses using interviews and reports from parents and teachers.

At what age can ADHD be diagnosed?

Signs usually start before 12. Specialists are cautious under 5, because high energy and short attention are normal then.

Can girls have ADHD?

Yes. Girls more often have the quiet, inattentive type and tend to be noticed later.

Does screen time cause ADHD?

No, but heavy evening screen use can disturb sleep and make attention and mood worse in any child.

Does sugar cause hyperactivity?

No. Excitement at parties is usually the setting, not the sugar.

Will my child grow out of ADHD?

Some symptoms, especially hyperactivity, often ease with age, while attention difficulties may continue. Support helps at every stage.

Who should I see for ADHD in my child?

Start with a paediatrician, who can refer you to a child psychiatrist or child psychologist.

Can poor sleep look like ADHD?

Yes. Tired children are often restless and unfocused. Snoring and late bedtimes are worth checking first.
